APPLICATION

Applicant, most respectfully request for the approval to adopt its own Color Scheme for Taxi
Cab service and, in support thereof, states that:

2. Considering the number of authorized units owned and under the operational control of the
applicant, it can already be considered as a taxi fleet operator, thus, qualified to adopt its
own color scheme;

3. Applicant will comply with all the requirements of this honorable board and will abide with
the existing LTFRB rules and regulations, the public service act, and other pertinent /
policies.

4. Applicant has completed the necessary documentary requirements for the Application,
copies of which are hereto attached in Annexes.

5. With the consideration and approval of this Application, public necessity and convenience
will be promoted in a suitable manner.

WHEREFORE, it is most respectfully prayed that (a) a Notice of Hearing be issued, and
(b) This Application for Adoption of Color Scheme be accepted, considered and approved.
