and Stakeholders

1. In line with the implementation of Republic Act. No. 10533, otherwise known as Enhanced Basic
Education Act of 2013 (K to 12 Law) and the DepEd Order No. 36, s. 2016 or the “Policy Guidelines and
Recognition for the K to 12 Basic Education Program”, the school is hereby announced to conduct School
Quarterly Awards and Recognition For Learners, Teachers, Parents/Guardians, and Stakeholders.
2. These awards have been designed to formally recognize the outstanding performance, achievements
and contributions of Learners, Teachers, Parents/guardians, and Stakeholders in the school.
e. Summative Tests should be submitted a day before the administration with Table of Specifications
checked by the Master Teacher assigned:
2. Summative test should be aligned also on the subject being taught. (Based on the Class
Program)
3. Applied the updated Blooms Taxonomy of Objectives.
B. Professional Development:
a. Enrolled in any Graduate/Technical/ Vocational Courses
1. Proof of enrollment

b. Mentoring/ Coaching
1. Proof of Attendance (PMCF)
c. Seminar attended (any of the following)

a. Open Time Clock should be properly utilized. Actual time should reflect in your respective

c. Logbook- Properly accomplished with Mid- Time and signed by the teacher. (8:00-12:00-
1:00-5:00)
d. Grade Level Attendance
2. Follow the AWA Schedule
b. Wearing of Complete Uniform with School ID, closed shoes every Monday- Thursday.
c. Wearing Descent clothes with School ID every Friday.
d. Accomplished Health Declaration (*Use the Online for faster tracking/checking) for teachers who are
reporting to school physically.
G. Submission of Reports:
a. DTR- 2- day before the end of the month
b. Monthly Enrollment Report - First day of the Month
