VENUE: ER (No specific place, look for Sir RJ)

2 Kinds of Patient:
1. New Patients
a. Charts will be at the Nurse Station
b. Complete Hx and PE
c. Jotform JOTFORM - ABC
d. Fill-up Form
e. Give Vaccine (PCECV, ID)
i. ERIG
ii. ATS/TT
2. Patient for follow-up
a. Ask for the follow-up form, check
their schedule
b. Check their name sa list ni Sir RJ
c. Write their names sa logbook for follow-up
d. Give Vaccine (PCECV, ID)

ER-TRIAGE
- When not sure what to do (Decking/Problems with Residents/Patient), always call
the attention of the MOIV on duty (Direct phone line to MOiIV on duty, is always
available on the triage disk.
- Join the ER-Triage group chat and be mindful of any updates. (Pwede
magpareliever ng co-intern if maglulunch)
COMMUNITY RODRIGUEZ

ZUMBA SCHEDULE: Monday & Friday at 8am

● Bring stethoscope & bp app - take participants bp before zumba session
● Always bring community logbook to log their names and bp monitor

CLINIC SCHEDULE: Tuesday & Wednesday at 8am

● Dalhin lagi yung medicine box at yung clear envelope and black envelope for lab
requests; make sure na yung prescription pads and malasakit forms are always
available; double check din lagi kung may kulang na requests para marefillan.
● Don’t forget to bring community logbook nakalagay BRGY. 140 RODRIGUEZ
○ Log patients name, age, sex, new patient or old patient and diagnosis
● Print your own SOAP forms or have your own interview sheet.
● Update census:

○ Pag may pedia patient seen, may separate na census sila depende sa age
● Patients for medical certificate for medical assistance:
○ Pagbalik from community, make sure na nasa ospital na rin si patient para mag
pa-in sa opd.
● Gawin ang chart nya, print 2 copies pa rin; 1 for records at yung 1 para copy natin na
ilalagay sa clearbook.
● Print patient's chart para ma-check din ng RIC.
● Don’t forget mag document ng mga ginagawa.
● Health teaching- Before magstart ang consultation
○ Topics: given by RIC or from DOH calendar
○ Prepare a ppt presentation and pamphlet (20-25 copies)

COMMUNITY CANOSSA

● Start: 8AM
● Health Teaching
○ Topics: given by RIC or DOH calendar
○ PPT and pamphlet
● Cut off: 10AM
● Number of patients: 20 to 30
● Always tally (census)
