(of Non-Operation of Business)

I, ALEX A. UGYU, of legal age, Filipino citizen, and residing at Brgy.

Remedios, Lubao, Pampanga, after having been duly sworn to in accordance with
law, hereby declares:

1). I am the registered proprietor/owner of AAA Hardware with business

address at Brgy. Remedios, Lubao, Pampanga;
2). That my hardware business was issued Mayor’s Business Permit,
Business Plate No. 4025, and the like, by the Municipality of Lubao, Pampanga;
3). That my business is engaged in selling hardware products and
construction supplies to the buying public;
4). That on March 27, 2017 I started non-operating my hardware business
until at present. The reason of non-operation was due to financial losses, fading
and incurring negative sales;
5). I am executing this affidavit, for all legal intents and purposes, regarding
the circumstances of NON-OPERATION of my hardware business since March
27, 2017 up to present time.
