reason

a. Subjective data
Answer : - Biographic Data
- Chief complaint or reason for visit
- History of present illness (Current health status)
- Past history
- Family history of illness
- Life style
- Social data
Those information are subjective data because we receive from the patient or from
one of his or her knowledgeable companions

b. Objective data
Answer : - Review of system
- Patterns of health care
- Psychological data
Objective data we can observe using our senses, it is measured and observed
through vitals, tests, and physical exams.

2) Make dialogue by using the terminology

Dialogue 1 Objective
ulfa ( patient mom ) : “ doc, what happend with my daughter doc?”
siti ( doctor ) : “ calm down mom,your daughter has diarrhe ”
ulfa (patient mom) : “ how is the right action for my daughter doc? ”
siti ( doctor ) : “ your daughter must be hospitalized, so that your
daughter not lack of fluids and need more intensive care”.
Ulfa (patient mom) : “ okay doc, do the best for my daughter”

Siti (doctor) : “okay mom”

Doctor call is nurse 3..

Siti (doctor) : “ nurse, please move… to the jasmine room. After that give
infusion fluid therapy and give Loperamide ”
vhya (nurse 3) : “ okay doc ”
(nurse… move… to jasmine room)

Vhya (nurse 3) : “good afternoon, my name is nurse vhya can you tell me
about you name and age? ”
Meifa (patient) : “ my name is meifa… I am 15 y.o”.
Vhya (nurse 3) :” okay.. I will do installation of infusion for replacing lost
fluids, how.. are you ready? ”
Meifa (patient) :” okay nurse, I am ready“
Vhya (nurse 3) :” okay (nurse 3 do installation of infusion).
(5 minutes later)
Vhya (nurse 3) :” okay, I am done to installation of infusion and you can
take loperamide for diarrhea”.

Ulfa (patient mom) : “okay nurse”

Vhya (nurse 3) : “ if you need me you can call me in the nurse’ s room mom
Ulfa (patient mom) : “yes mom”
The next day the doctor visited the inpatient room.
Siti (doctor) : “good morning”
Meifa (patient) : “morning doc”
Siti (doctor) : “how are you feeling now? Do you still feel limp and often
diarrhea”

Meifa (patient) : “alhamdulillah, I am not limp and diarrhea doc”

Siti (doctor) : “ okay you most routine of take medication”
Meifa (patient) : “ okay thank you doc ”
Siti (doctor) : “ you are welcome, I want to go back to my room, bye”
Meifa ( patient) : “ bye doc”

(patient was treated for 3 days in the hospital, after that her can go home)
(mother patient go to receptionist for payment)

Ulfa (patient mom)m : “ excuse me, I want to complete my daughter

administration “
Vhya ( receptionist) :” okay, what is her name and her room?
Ulfa ( patient mom) : “her name is meifa.. in the jasmine room ”

Vhya (receptionist) : “the total is 900.000 (with give the letter). ”

Meifa (patient) : ”okay thank you”

Vhya (receptionist) : “ you are welcome.”

Dialogue 2 (subjective)

Jihan (doctor) :”good morning,can let you know with anyone”.

Ghina (patient) :” my name is ghina, doctor”

Jihan (doctor) : “ghina now what age”

Ghina (patient : “ I am now 19 years old doctor “

Jihan (doctor) : “ what made ghina come to go hospital”

Ghina (patient) : “ I’ve had a fever two days with bowel movement five
times a day”

Jihan (doctor) : “the color of defecaling like what”

Ghina (patient) : “ the greenish yellow color mixed mucus “

Jihan (doctor) : “ besides shitting and fever, what else to complain about

Ghina (patient) : “ I also puked up to twice a day”

Jihan (doctor) : “have you had any treatment”

Ghina (patient) : “ 2 days ago was brought to the us susifa medical treatment
hall”

Jihan (doctor) : “has this happened before? What family and environment?

Ghina (patient) : “ before having diarrhea eight times a day every two hours,
others familiers had the same experience, and we lived in a dusty, dense
neighborhood “

Jihan (doctor) : “ alright thank you for this cooperation”

Ghina (doctor) : “ thanks back doctor”.
