Directions: This form shall be accomplished during the conference between the instructional
supervisor and the teacher. Additional classroom observation-related information may be
requested to serve as guide during the classroom observation proper. A copy of this
accomplished form should be given to the teacher.

Differentiation activity to be used
Group I & II will color the circle according to the phase of the moon, group III & IV
will arrange the illustrations in chronilogical order and group V and VI will color
and places the phases of the moon in the illustration given.
8. Learning Environment
Values integration for safe learning environment (fairness, respect, care, collaboration,
responsibility) to be integrated
The teacher reminds pupils to show respect to the person talking, to work collaboratively
during group activity and to show care and fairness to everyone in the class.

Process
Existentialism and constructivism- Pupils have different levels of intelligence, thus
differentiated activities should be given to suit the needs of the pupils. Pupils should also be
given the chance to speak regardless of their level of intelligence. Praises are also important to
encourage to learners.
