POSSIBLE REASONS OF FAILRUES:-

 Observation and nature of failure it noticed that lack of lubrication &
Ovality founded in chocks.

PREVENTIVE ACTIONS:-

  Maintain bearing seating size as per drawing.
 Maintain chock bore dimension zero to plus 20 micros.
 Use Coper and brass tools for bearing mounting.
 Ensure proper bearing clearance as pe OEM for respective bearings.

GENERAL RECOMMENDATIONS: -
 Lubricate bearing till grease purging.
 Ensure all dimensions of chocks should be checked.
 Ensure misalignment.
 Ensure vibration.
 Ensure lubrication
