Chief Complaints: cough and fever Room: 56A
Diagnosis/Impression: Community Acquired Pneumonia, Moderate Risk with Pleural Effusion

HEALTH TEACHING

1. Teach and assist the patient in performing proper deep-breathing exercises.


2. Teach patients and SO members correct coughing to loosen mucus.
3. Teach proper nutrition and hydration.
4. Teach the importance of increasing fluid intake and limiting fluid intake in the
evening if nocturia occurs.
5. Instruct patients and SO to strictly follow physician’s prescription when it comes to
medication.
6. Instruct patient and SO to always check lower extremities for any wounds and to
notify the doctor if there’s any.
7. Instruct a patient to contact her physician right away if breathing gets worse.
8. Teach patients and SO that lying with your chest lower than the belly (abdomen) can
help loosen or drain remaining mucus from the lungs.
9. Drink warm water or use warm water in taking a shower to ease breathing.
10. Instruct the patient to use a mask to protect herself and the people around her from
infection.

ACTIVITY

● Perform active and passive range of motion exercises.

● Light exercise such as walking to help the lungs to regain strength.

● Advice to take a rest when tired

● Advise patients to contact the doctor if there is any complication after doing these
activities.

OBSERVABLE SIGNS AND SYMPTOMS OF COMPLICATION

● Evaluate or continuously monitor physical state and condition

● Encourage urgent consultation if any of the following symptoms appear.

NUTRITION

● Diet as tolerated:
1. Low salt
a. Use spices, herbs, and salt-free seasonings instead of salt
b. Avoid packaged, processed, and fast foods
2. Low fat
a. Leafy greens (lettuce, bok choy, cabbage, malunggay, broccoli)
b. Fruits (apple, banana, pineapple)
c. Sweet potatoes (kamote)
d. Lean Meat
e. Chicken Breast
f. Low Fat Dairy
g. Egg white
3. Moderate Protein Diet
a. Lean meat
b. Chicken
c. Fish
d. Corn
4. High Fiber Diet
a. Beans
b. Avocados
c. Wheat Bread
d. Whole Grains
● Limit liquid intake.

● Follow the doctor's order regarding the prescribed diet.

● Note total daily intake, including patterns and time of eating.

● Eat with moderation.

ENVIRONMENT

1. Emphasize a smoke free environment.


2. Encourage patients to stay away from areas infested with smoke to provide the
lungs enough time to heal.
3. Check the patient’s environment for hazards that may cause the patient’s
wounds and remove the hazardous objects from the bedside.
4. Instruct to maintain cleanliness in the environment.
5. Encourage a peaceful and allergen-free environment.
