INTRODUCTION

Considering that 1 in 7 people with hypertension do not have their blood pressure

controlled, identifying, treating, and controlling hypertension must become public health

priorities. Even after being diagnosed, there are many barriers that make it difficult to live with

and manage hypertension, such as complicated treatment protocols, inability to afford

medication, and lack of time to return to the health clinic for follow-up visits. One of the key

recommendations of the updated WHO hypertension guideline is patient-centered care. To meet

each patient’s needs, a differentiated service delivery approach is recommended. Controlling

blood pressure should be easy, quick, and affordable for the patient, with blood pressure

screening convenient to their home and single-pill combination (SPC) medications with multi-

month refills. Hypertension is a major public health problem and a leading cause of death and

disability in developing countries. Hypertension and its cardiovascular complications are major

contributors to the global burden of disease and account for 8.1% of disability-adjusted life-years

in the less developed world.

In the Philippines, CVD is the leading cause of morbidity and mortality, with

hypertension identified as one of its risk factors. Hypertension has become the most common and

preventable risk factor for CVD.2,3 Poor control and inadequate treatment of hypertension has

led to an increase in the incidence of hypertension-related complications such as stroke (ischemic

or hemorrhagic), ischemic heart disease, vascular diseases and chronic kidney disease. Due to

this, non-communicable diseases continue to be the top killers both in the Philippines and

worldwide. Of these, hypertension remains to be the leading illness while with diabetes is at par,

affecting around 5% of our adult population. These diseases are chronic and irreversible.
The Bangsamoro Autonomous Region in Muslim Mindanao (BARMM), located in the

Philippines, is of particular interest due to its unique cultural and socioeconomic characteristics.

In 2020, hypertension was the leading disease in the Bangsamoro Autonomous Region of

Muslim Mindanao (BARMM) of the Philippines, with cases amounting to approximately 4.3

thousand.

GENERAL OBJECTIVE

After spending 4 days, the nursing students from Notre Dame University are able to recognize

the health problems and challenges that exist within the community. As part of their

responsibilities, the nursing students must gather and examine essential information, determine

the most critical issues affecting the assigned community, and present a full community

diagnosis to the relevant governing bodies of the city and different barangays.

SPECIFIC OBJECTIVES

 Collect relevant data to establish a baseline of community information and resources.

 Analyze the current health status of the community to identify areas of concern and

potential health problems.

 Identify health issues that exist within the community.

 Create short-term and long-term plan of action or activities that support the health and

safety needs of the community.

 Prepare an organized and comprehensive presentation of the consolidated output of the

community assessment and diagnoses.


 Use the presentation to communicate the findings of the community assessment, the

identified diagnoses, and proposed interventions.

DEMOGRAPHIC PROFILE

The total population being profiled consists of 203 individuals who resides in different barangays

here in Cotabato City. The population is diverse, with individuals of different ages, genders, and

ethnicities represented.
