    Sir:

    I am writing this letter to inform your office that I, GLENN SALVADOR B. LIM IV, a
    participant of the three-day seminar workshop (November 17-19, 2023) at the Regency Hotel,
    Calasiao, will not be staying at the hotel accommodation intended for us. I opt to go home
    every after sessions in the afternoon because my family needs me especially my wife as she is
    the only one taking care of our daughter a 7-month-old infant. Hence, the need for me to be
    there and help in nursing our daughter.

    At any rate, rest assured of the completion of whatever output is required of me being one of
    the participants.

    I, GLENN SALVADOR B. LIM IV, hereby waive the Urdaneta City Division training teams for any
    injuries or damages that may occur during my travel time to and from the venue.
