b. Post Mortern report I

c- Death certificate & legal heir declaration certificate issued by a

competent revenr-te authorities.

Prirnary Sheep Breeders Cooperative sociegr arrd. attested by the
concerned veterinarian mentioning the registration nurnber of the
mernber in Primary Sheep Breeders cooperative society Ltd..
e- Ration card or arly other photo identity card in -uppo.i of the claim
2. The proposal should reach the Joint Director (A.H) through proper
channel i.e concerned vAS and Divisional Assistant Director-(A H),
rvith their recommendations that- the clairn is as per the spirit of the
G.O.

7. PROCEDURE FOR PAYMENT OF E](- GSIATIA:

1. The Joint Director {A.H) shall examine rhe proposal and take the
approval of ttre District Collector for considcr-ation
2. Necessary sanction shall be given by the Drstrict. collector and
account payee dernald draft shall be obtained. and disbursed through
thb concerned Divisional Assistant Direcror (A.H)
3. Expenditur'.-, shall be incurred from the funds provided und.er sheep
insurance scherne in the respective districts.
8- RECORDING, REPORTING AND ACCOUTING:

1. Each case shalt be properly recordecl, accounted er.nd reported to the

head office
2- Separate register shall be maintained for irnplernentation of the G.O.
