7 Preparing a Portfolio Assessment Plan

Objective: The students should be able to prepare a portfolio assessment plan

Instructions:
1. With your group, choose a subject taught in the elementary grade. Decide on a
unit of instruction (quarter).
2. Specify the Purpose of the portfolio. Identify the learning targets to be assessed
and the proposed use of the portfolio
3. Describe the physical structure of the portfolio that you want your students to
accomplish.
4. Let us assume that you will be the one who will determine the evidences of
learning students will include in their portfolio. List them under Content. Identify
1 or 2 evidences for every learning target you identified
5. Specify the Self-reflective guideline
6. Specify the scoring criteria. For each content/artifact

I. Purpose

Learning Targets to be assessed (Edit)

1. classify objects and materials as solid, liquid, and gas based on some observable characteristics;
2. describe ways on the proper use and handling solid, liquid and gas found at home and in school;
and
3. describe changes in materials based on the effect of temperature:

Proposed Uses:

A. to document achievement for evaluation purposes (Edit)

B. to document growth and improvement over time towards the standards

C. To document evidence of range of student work.

II. Physical or Digital Structure

You are expected to construct a portfolio made by either a bond paper or a colored paper depending
on your creativity. This can be in a form of printed or written portfolio with a dimension of a short
size bond paper. Outputs online can be printed along side and written outputs can be paste to the
specified bond paper. Arrange your entries in chronological order.

Cover Letter or Letter to the Reader

Table of Contents

Content

Reflection

Teacher Feedback/Comment page

III. Content (sample)

1. create a chart classifying objects found at home into solid, liquid or gas

2. Make a comic strip showing the proper use of solids, liquids and gas found at
home/ create a video of you demonstrating the proper use of solids, liquids and
gases found at home. Create a google file for your video and copy and paste the
link of the google file in your portfolio.
3. conduct an experiment applying different levels of temperature to solids, liquids
and gases. Conclude how temperature turns

V. Scoring Criteria

A. Criteria for Portfolio Structure Evaluation

Appearance – Creativity, thoroughness, artistry, and neatness

Organization- required components are included, properly sequenced, and


clearly labeled.

Content - scoring criteria and rubrics will be used to evaluate each


individual entry in the portfolio.

Reflection- will be judged by clarity and depth of thought, the level of


analysis, and the clarity of communication.

B. Criteria for Evaluation of Individual Entries

1. Entry No. 1-will be evaluated based on the accuracy of


gclassifying objects into solid, liquid or gas

2. Entry No. 2- will be evaluated based on the appropriateness of


using solids, liquids and gases.

3. Entry No. 3 -will be evaluated based on the accuracy and

comprehensiveness of the conclusion
