Medical implants including spinal cord simulators used for pain
management are classified as medical devices and fall within the scope
of products regulated by Botswana Medicines Regulatory Authority
(BOMRA).However, due to lack of capacity at BOMRA, the authority
has not yet initiated the full registration of medical devices. Minister
of Health, Dr Edwin Dikoloti explained to Parliament that due to
capacity limitations BOMRA has since inception in December 2018
prioritised its focus on development of structures and process.“This
is in part due to limitations in the law which is currently under review
and once the legislative frameworks permit, all medical devices
used in the country will have full regulatory coverage for effective
oversight,” he said. Dr Dikoloti said since the spinal cord simulator
was not registered with BOMRA, its quality, efficacy and safety had
not been fully established.“In the meantime, BOMRA has completed
the process of listing medical devices that have been in Botswana
which will ultimately lead to registration,” he explained. He, however,
said the authority listed all devices used in the country and have been
published on BOMRA website, and that al the listed medical devices
were subjected to regulatory processes such as vigilance, quality and
safety reporting, post market surveillance as well as export and import
controls. Dr Dikoloti also explained that spinal cord simulators were
used in the management of chronic pain and could treat a wide range
of pain types from different conditions. He said the device could
also be used to treat pain that had not responded to other types of
treatments.“There are several types of these devices depending on the
manufacturer but the mode of action is generally the same as each
simulator uses electrodes to send electrical impulses where the pain
