Worthy Sir
Most respectfully I beg to submit my humble submissions for your
kind consideration:-
a) That two months' leave on a medical basis (Coronary Artery Disease- CAD) has
been allowed to the undersigned vide notification No A/1057/HR-1/2023/6142-51
dated 04-09-2023.
b) That during this period, I have improved my health with medication and care
however frequent changes in Blood Pressure again caused subconjunctival
hemorrhage which needs my attention as I am the sole bread earner of my entire
family. The present ECG, last month's personal BP chart, Lipid profile (improved),
and follow-up notes from the cardiologist (Punjab Institute of Cardiology Lahore) are
enclosed for your kind consideration.
c) That in case of extreme state necessity, the applicant will join the duty during this
extended medical leave.
It is therefore requested that a 20-day extension in medical leave please be
allowed. I shall be highly grateful for this act of kindness.

SUBJECT: APPLICATION FOR CHANGE OF SESSION ON
SYMPATHETIC GROUNDS

Most respectfully I beg to submit my humble submissions for your
kind consideration:-
a) The applicant is serving officer of the Prime Investigation Agency (FIA) of this
country and leads the law department as a senior law officer. Furthermore, the
applicant represents the agency in Hon'ble higher courts and also conducts the
prosecution of all major cases.
b) The applicant is presently posted at FIA HQ Islamabad and his family is living in
Zarar Shaheed Road, Lahore Cant.
c) My daughter EMIN CHOUDHARY is currently enrolled in BS
ENVIRONMENTAL SCIENCES 1st semester (Evening Session) on a self-support
basis which is under your kind control.
c) My elder daughter Manahil Choudhary is also studying BS-Gender Studies (6th
semester) Morning session in this prestigious institution.
d) That my residence is far away from campus and due to different sessions of
studies, we as parents have to long wait for their arrival which causes stress to us.
Furthermore, your good self as a renowned educationist is also well aware of the
present state of affairs of the country.
In light of the above-mentioned submissions, it is therefore requested
that my daughter (Emin Ch) be allowed only to attend the morning
classes/session (On self-support fee) on sympathetic grounds.
Your kind action will provide me a sigh of relief and boost my energies for
providing better services to the nation of this beloved country.
