Introduction

Mathematics in the Modern World (GECC 103) is a general education elective core
course that deals with the nature of mathematics, appreciation of its practical, intellectual,
and aesthetic dimensions, and application of mathematical tools in daily life. One such
application of mathematics is on the use of statistics. To experience first-hand how powerful
a tool statistics is, the students enrolled in this course have been tasked to organize, present,
and analyze data collected from DMMMSU students.

Participant Selection

As one of the enrolled DMMMSU students, we would like to invite you to take part in
this survey as part of the performance task of the students enrolled in the subject GECC 103.

Voluntary Participation
Your participation in this survey is entirely voluntary. It is your choice whether to
participate or not. The choice that you will make will have no bearing on your scores or
grades in school. You may change your mind later and stop participating even if you agreed
earlier.

Procedures
We are inviting you to take part in this survey. Your responses will help the students
enrolled in the subject GECC 103 in finishing their performance task. If you accept, you will
be asked to fill-out a survey-questionnaire. This will be given to you by the students, and it
will be collected after a few minutes. We will be the ones who will investigate and analyze
your responses.
If you do not wish to answer any of the questions in the survey, you may skip them and
move on to the next question. Rest assured that the information recorded will be handled
with utmost confidentiality and will be used for academic purposes only. No one else except
us will have access to your responses in the survey.

Duration
The survey-questionnaire will take about 2-5 minutes of your time. You can answer the
form once you receive your copy from us and submit it afterwards.

Confidentiality
Your information will be stored and recorded and can only be accessed by us, the
researchers. This will be kept private, and we will not be sharing it to anybody. Your
responses will be treated with utmost confidentiality.

Sharing the Results

Your answers in the survey and all the information you will provide will not be shared
with anybody and nothing will be attributed to you by name.
Right to Refuse or Withdraw
You do not have to take part in this survey if you do not wish to do so and choosing to
participate will not affect your grades in any way. You can cancel your participation in this
survey anytime. You may leave without further explanation if you would want to. If you
decide to continue, we will be giving you the opportunity to review your answers, so you can
modify or remove parts of it before we encode the results.
