AFFIDAVIT

I, Camar Bandrang of legal age, Filipino and a resident of

BLK 13 LOT 37 CARMELO HOMES, PUAN, DAVAO CITY hereby depose and say, THAT:

• I am an employee of OFFSOURCING PHILIPPINES INC., with principal address at

GF TAVERA BUSINESS CENTER, PARDO DE TAVERA BRGY, 9-A, DAVAO CITY;
• On October 09, 2023 , I entered into an engagement with the corporation.

• On November 09, 2023 , I ended my engagement with the said corporation.

• Due to Termination, OFFSOURCING PHILIPPINES INC., paid me my last pay

by reason of my professional engagement with the said corporation at the amount of
(Php7,863.73).
• Aside from the above-stated benefits, I have also received money due to me, as
mandated by the Labor Code, Existing Laws and other applicable City Ordinances from my
engagement with OFFSOURCING PHILIPPINES, INC.
• I am releasing OFFSOURCING PHILIPPINES INC. from any liability under the
labor code and other related laws and Ordinances, in relation to my professional engagement
with it;
• I am executing this instrument to attest the truthfulness and correctness of the
above stated facts to the best of my personal knowledge.
