    At around 2030 hrs on August 15, 2018, Marlon Velarde was standing on the last step of a ladder while adjusting cable ties above the ceiling in Plant 1's Raw Stock Area. A safety officer, Paulo Martin, saw this unsafe act and was about to intervene when another worker, Arvin Baula, entered and witnessed the situation. Paulo Martin then corrected the workers and instructed them to use an H-frame scaffold instead of a ladder for safety. Corrective actions included using H-frame scaffolding instead of ladders for overhead work and conducting a safety orientation on proper work processes.

    NEAR MISS REPORT

    Time : at around 2030 hrs

    Date : August 15, 2018

    Name of person involved : Orly Valenciano and Marlon Velarde

    Scope of Work : Installation of feeder lines for 3 PACU.

    Exact Location : Plant 1 Raw Stock Area

    At around 2030 hrs., our workers at P1 raw stock area is adjusting the cable tie for the prior activity of
    fixing the feeder wire above ceiling. Marlon momentarily stand on the last step of the ladder to adjust
    the cable tie. At 2031 hrs., our safety officer, Paulo Martin, was about to apprehend the situation when
    Arvin Baula coincidentally walked inside the room and stayed for a minute to give advice about the
    situation. With this regard, our safety officer, Paulo Martin, corrected and oriented the operation and
    demanded an H-Frame to use instead.

    Corrective measures to be done is to use H frame scaffolding instead of ladder in working above ceiling
    and conduct a safety orientation regarding this work process.

    We are sincerely apologize for this matter and rest assured that we will keep remind and provide proper
    orientation to our team from this point of time.
