Fatal Injury-( Late Sh.

XYZ) AT BHARAT POWER GENERATION COMPANY

SAFETY CONCERNS: SAFETY IS EVERY BODYS RESPONSIBILITY

The Stage-I 3X500MW, Supply and Commissioning of Steam Generator


and Auxiliaries awarded to M/s A and for
Erection work of same has been awarded to M/s B , further for painting work
M/s C Enterprises engaged as sub agency by M/s B.

The accident took place on C at about 06.30pm when the


deceased person, Shri XYZ, Helper, age 19 years and other
six co-workers were engaged for painting work at U#3, ID Fan-3A Suction
Duct area, at aprrox. 20 mtr. Elevation.

Occurrence finding:
1. Painting work was planned at 6:00 pm on Occurrence finding:
1. Painting work was planned at 6:00 pm on 27.03.2012, for the painting of
ID Fan 3A Inlet duct and deployed the group of seven workers and Shri
XYZ, Helper as one of them.
2. To reach up to the actuator of Inlet gate are, permanent staircase is
already erected but no access to cross damper to go to the back side.
3. For the painting work, scaffolding pipe with proper clamping has been
provided to approach up to the work area.
4. On 27.03.2010, in the evening at around 06.30pm the Shri Late Shri
Jivrakhan Yadav with five other co-workers reached to work spot by
using permanent erected staircase near the suction inlet gate of ID
fan 3A, then tried to cross the motorized actuator shaft area to reach to

the work location which was not a safe access. And one helper was at
ground floor to assist for lifting painting material.
5. The working crew was not adopted the safe approach to go the work
place and used shortcut path.
6. Shaft for operating the Inlet gate was not guarded (No provision of
guard as per the available drawing).
7. One tor rod (approx. 20mm dia, 500mm length) was welded at left end
portion of the Shaft (while facing to wards chimney)
8. Commissioning activities of U#3 was going on.
9. For commissioning activities protection checking of U#3 boiler, ID fan
3B was started from CCR.
10. As per information gathered painting work is supposed to be
carried out at outer surface of duct hence permit was not
obtained.
Note: No concerned person of M/s C enterprises was available for enquiry.,

From the available information, interrogations and photographs taken just


after incident and examining the spot of accident, following is concluded by
the AIC:
i. Late Sh. XYZ while crossing the operating
shaft of inlet gate the lanyard of his full body harness was got
entangled with the rod welded on the shaft.
ii. Suddenly Inlet gate of ID Fan 3A start closing (as per auto
interlock system of starting of ID Fan 3B) and due to rotation of
shaft, victim lanyard got entangled with the rotating shaft then his
body was also start rotating and due to hitting of his head and
other parts of body with the platform and damper casing and he
succumbed to injury.

Estimation of Losses:
The Committee has also probed in to the extent of losses to
A) The Injured, b.) Machinery, c.) Man-hours and any other.
a. Loss of life of one person.
b. No loss of machinery.
c. Temporarily for one day as the work was suspended in that
location

RESPONSIBILITY
Responsibility of the employer (BHARAT POWER GENERATION COMPANY
) is to assert agency for
adequate supervision of construction work in the establishment under its
control so as to ensure safe working practices and to take all practical
steps necessary to prevent accidents.
However, looking into the nature of accident, visit & verification of accident
spot and detailed interaction with various section of concerned people, the
committee is of the opinion that,
H.(1). M/s. B & A, being the main agency for erection of Suction
Duct of ID Fan-3A erection and structural work as a part of U#3 Boiler,
Stage-I, 3X 500 MW. They are responsible for close Supervision and
ensuring safe work practices at site.
Recommendation by the AIC
1. All the Rotating and Moving parts where unsafe condition
persists are to be guarded.
2. Once the equipment is commissioned required permit to work is to be
obtained for any type of work in that area.
3. Proper guidance and training is to be given to the worker for using safe
approach & not adopting the shortcuts.
4. Continuous, close supervision and risk assessment of work to be
ensured while working at height.

The full-body harness is an important component of personal fall arrest systems keeping workers
suspended upright in the event of a fall, and supporting them while they await rescue. In many cases, fullbody harnesses can also be used in fall restraint systems which prevent workers from reaching points
where falls can occur. Most regulations prohibit the use of body belts for fall protection due to the
concentration of fall forces on the abdomen. In the event of a fall, full-body harnesses distribute fall forces
throughout the body, substantially reducing chances of injury.
