AFFIDAVIT OF PARENTAL CONSENT

We, Jerry Go Tiu and Jhoanna Apostol Tiu, both of legal age, Filipino Citizens, married to each
other, with postal address at Pres. Quirino St. Brgy.31 Sto. Niño, Cagayan de Oro City, after having duly
sworn to in accordance with law hereby depose and say:

That we are the parents of Kurt Ulric Apostol Tiu (son born on December 18, 2004), who is living
with us at Brgy. 31. Sto. Niño, Cagayan de Oro City;

That we are giving consent to our son, Kurt Ulric Apostol Tiu, to apply for a Non-Professional
driver's license;

That we have executed this affidavit to attest to the truth and veracity of the above stated facts
and circumstances for whatever purpose it may serve best;

That we have executed this affidavit freely and voluntarily.
