1. Introduction
The Mumbai – Ahmedabad High Speed Rail (MAHSR) Project is the first High Speed Rail (HSR)
network planned to be constructed in India. The MAHSR Project line connects Mumbai, located
in Maharashtra State with Ahmedabad in Gujarat State, passing through one Union Territory,
Dadra and Nagar Haveli (refer Figure 1). The entire stretch is divided into 8 packages from C1 to
C8.
C4 Package of High Speed Rail Project is awarded to L&T Construction. This package consists
of four stations namely, Vapi, Billimora, Surat and Bharuch and other structures viz viaducts,
SMD, Ramp, etc. C4 package starts from Ch. 156.6 km at Zaroli village to Ch. 393.7 km which
ends near Vadadora. It covers approximately 237.1 km in this package. The alignment of the
MAHSR C4 package is shown in Figure 2.

2. Scope
• Form work: This work shall consist of furnishing and fixing of form work(shuttering) of approved
quality for cast- in- situ / pre-cast plain or reinforced concrete with necessary props, staging,
centering, scaffolding and any temporary construction required for their support.
• Reinforcement: This work shall consist of furnishing and placing of high yield strength deformed
bars of the shape and dimensions as shown in approved drawings.

3. Terms and Definitions


• Employer- Shall mean “National High Speed Rail Corporation Limited”.
• Contractor- Shall mean Larsen & Toubro Limited.
• Engineer- Shall mean the person / firm appointed by Employer to audit and review the Project on
behalf of employer.
• Agency- Shall mean the “Agency” appointed by the Contractor to perform the survey.
• Works - Shall mean and include the design and build of all necessary civil works, rolling
stocks and systems to enable the Project to commence operations.
• Accomodation Work Areas- Shall mean the proposed land available for the Works.
• Hold Point- A stage in the process beyond which point work shall not proceed until the
designated person being given the appropriate notice to inspect such Works or activity
through performance of inspection and/or testing to determine the acceptability of an item for
further processing as set out in the related quality procedure.
• Witness Point- A stage in the process when a third party inspector, Contractor or PMC
representative witnesses any inspection and or test and work can proceed further without
signoff by witnessing authority.
• Review- A stage in a process, when a third party Inspector, Contractors or PMC representative may
verify records and or procedures and determine the acceptability of the same.
• IRS CBC- Indian Railway Standard Code of Practice for Plain, reinforced and Prestressed
Concrete for general Bridge Construction.
• IRC- Indian Roads Congress is the apex body of Highway Engineers in the country.
• QA- Quality Assurance are actions taken to design and manufacture a safe and effective product
by building quality controls into the product life cycle.
• QC- Quality Control are test procedures used to verify that a product is safe and effective after
manufacturing is done.

5.1 Section-In-Charge:
• Reports to Project Director

shifts
• Responsible for good industrial practice to place the concrete with a suitable means that will not
segregate the concrete ingredients
• Deliver the work in qualitatively and quantitatively manner

• Raise RFIs and initiate inspections along with Customer

5.4 Production Engineer:


• Reports to Production-In-Charge
• Responsible to feed ingredients of concrete uninterruptedly
• Responsible to achieve the mix as per the approved mix design
• Ensure the temperature of mix within the specified limits
• Ensure the loading of mix into cleaned and water-free transit-millers
• Ensure transit millers are wrapped with hessian cloth during hot weather
• Responsible to adopt safe production/work practices
• Suggest and implement improvement to product quality

5.5 Site Engineer:


• Reports to Shift-in-charge
• Take action for all sub-activities in his area
• Responsible for arranging and attending all internal and Customer’s inspections against RFIs.
• Execute the shift related activities
• Cross check availability of resource before start of operations
• Follow latest approved drawings
• Follow approved work method statements and interact with QA-QC/Shift/Section-In-Charges if
any change or revision needed in the procedure that enhances the quality of construction
• Ensure the work front is adequately available
• Cross check the levels transferred on to the structure
• Responsible to achieve the required dimensions of structure
• Responsible for curing of concrete
• Interact with surveyor, QA/QC team and EHS team
• Responsible for safeguarding TBMs and centerline pegs/spikes

5.6 Surveyor:
• Report to Shift-In-Charge
• Ensure only calibrated survey instruments are used during construction
• Responsible for marking levels and fixing coordinates of the structure

• Responsible for marking of centre line of piers, abutments, piles and boundaries of foundations
• Responsible to mark alignment
• Responsible for taking levels before and after concrete placement and report to Section-In-
Charge and Shift-In-Charges on achievements and lapses.
• Responsible for safeguarding TBMs and centerline pegs/spikes
• Responsible for calibrating and recalibrate TBMs and centerline pegs/spikes.

5.7 QA/QC Engineer:


• Reports to QA/QC-In-Charge
• Ensure only tested and approved materials are being used in construction
• Responsible to sample and test all ingredients of concrete before commencement of days
production as per set test frequency
• Coordinate/Supervise with Site team for concrete works
• Responsible for plant calibration as per frequency
• Coordinate with Customer on quality related issues
• Arrange any regular tests conforming to specified frequency on materials, mix and finished
structure and/or specialized tests, if any
• Ensure shift hand over and takeover of the QA/QC activities
• Identify and raises non-conformances, in case of works/activities not meeting the specification
requirements
• Responsible to keep test reports in his purview, up to date
• Suggest and implement improvements to product quality

5.8 P&M Engineer:


• Reports to P&M-In-Charge
• Responsible for plant operation, availing the required fuel, oils and mechanical parts for smooth
running of plant.
• Responsible for site machineries operations like concrete needle vibrators, concrete pumps or
boom placers.
• Responsible for periodic & incidental maintenance of concrete plant and site equipment for
smooth and continuous operations.
• Attend incidental breakdowns immediately

• Ensure lock-out and tag-out system while attending breakdown and/or maintenance
• Suggest and implement, improvements to product quality

5.9 EHS Engineer:


• Reports to EHS-In-Charge
• Involve in regular pep-talks and periodical demonstrations of safety practices at plant area & site
• Help to provide necessary safety materials at the work location as per requirements
• Implement EHS system at plant & site
• Guide/Support the production & Execution team in identifying the entry-exit locations of
construction equipment and provision of sign boards and barricading wherever necessary.
• Coordinate and arrange flagmen at entry-exits of construction vehicles
• Ensure the site is illuminated sufficiently, in case of night work is carried out
• Identify/supervise safety lapses before the start of work and ensure the same are corrected
• Coordinate with site administration, first-aid, Doctor and hospitals during emergency.

6. Manpower

• The project will be divided into 5 sections and each section will have a project director.
• Manpower shall be deployed as per requirement.
• A safety steward will be available at every work location, so as to monitor all the working
machineries at a location.
• The total availability of manpower shall be submitted separately.

7 Materials
7.1 Reinforcement Steel
• High strength deformed steel bars for concrete reinforcement used in the works shall be
Fe500D/Fe550D TMT, conforming to IS 1786 and manufactured by SAIL/TATA
STEEL/JINDAL/JSW STEEL/RINL/IISCO.
• No rerolled steel shall be used.
• Reinforcement steel shall be stored as per IS 4082.
• The reinforcement bars shall be bent to conform to the dimensions and shape shown in the drawings
and Appendix -7, Division 2000, Technical Specifications of Employer’s Requirement. However,

any change in bar bending to suit construction feasibility will be followed as per approval from the
engineer.
• Bars shall be bent cold and any bent reinforcement shall not be re-bent. However, when it is
unavoidable to re-bend the reinforcement, the same shall have approval from the engineer.
• Placement of reinforcement shall conform to the provisions laid down in IRS - CBC (CL. 7.1.3)
(Refer Appendix-1). Cover and spacing of steel shall be uniform and as specified in the
specifications and as shown in the drawings.
• Uncoated reinforcement steel shall be protected from rusting or chloride contamination.
Reinforcements shall be free of rust, mortar, loose mill scale, grease, oil or paint.
• Cover blocks shall be firmly placed at appropriate intervals to maintain specified concrete cover to
the reinforcement. The number of cover blocks to be provided shall generally be 4 pieces per square
meter for the bottom surface of the member and about 2 to 4 pieces per square meter for the side
surface of the member. Cover blocks shall be made of concrete or mortar having quality equal to or
superior than that that of the parent concrete.
• Procurement of reinforcement steel shall be so phased by the contractor that the storage period
before its actual use in the works is limited to the bare minimum as directed by the Engineer.
• All reinforcement bars are properly tagged for easy identification.
• Tolerances & Criteria:
Tolerances and criteria for reinforcement work shall be as specified in Table 2.

• Lapped splices - Standard design drawings indicate the locations of splicing of bars. Other than
those, no splicing of bars shall be permitted without prior approval of the Engineer. Lengths of
splice, wherever required, shall be as indicated on the Standard Design drawings and/or shop
drawings subject to a minimum length of 40d for rebar diameter ‘d’ less than 25mm and 50d for ‘d’
greater than or equal to 25mm and 50d for 'd' greater than or equal to 25mm, approved by the
Engineer. Lapped splices shall be staggered and located at points along the span where stresses are
low, unless otherwise indicated in Standard Design drawings, other parts of the Technical
Specifications or approved by the Engineer.
• Mechanical Joints - Standard Design drawings indicate the locations of jointing of bars. Other than
those, no jointing of bars shall be permitted without obtaining approval of the Engineer. Quality
control of mechanical joints shall be as per IS 16172.
• Design Adjustment - Where the diameter of the reinforcement bar is not greater than 32mm, lapped
splice may be changed to mechanical joint or vice-versa, subject to the Engineer’s approval.
• GI Binding wire shall conform to the provisions laid down in IS 280 and IS 2502.

7.2 Formwork & False work


• The formwork shall be of steel plates of proper thickness to give good finish.
• Finishing shall conform to IRS: CBC (CL. 6.2.4 and CL. 6.2.5) (Refer Appendix-1).
• Formwork shall be made to produce a finished concrete true to shape, line, levels, and dimensions.
• Chamfers shall be provided at all angles of the formwork to avoid sharp corners. The chamfers,
bevelled edges and mouldings shall be made in the formwork itself, conforming to the drawings.
• Specialized formwork may be required in the case of slip formwork and underwater concreting.
Formwork shall confirm to the provisions laid down in IRC: 87 (CL. 10) (Refer Appendix-1).
• Refer to the method statement for Concrete Production, Transportation and Placement for detailed
procedure on concreting technology.
• Design and drawings of complete formwork with supports shall be submitted for Engineer's
approval.

8. Formwork (Methodology)
General-
• Formwork shall be designed in consideration of appropriate raising (camber) against sinking and
deformation due to the weight of the concrete during construction and after completion.
• Ties shall not be welded to the reinforcement bars with the exception of distribution reinforcement
and other reinforcements which do not contribute to structural performance. Clear cover to the end
of the ties shall not be less than 25 mm. Filling of tie locations after removal of formwork shall be
carried out with dry pack cement mortar.
Design-
• The foundation of all supports shall be designed to suit the bearing capacity of soil to support the
designed loads without settlement.
• Shop drawing showing the arrangement of formwork for structural members including shoring
system, horizontal and diagonal bracing system, details of foundation shall be prepared. The sizes
of individual members shall be as per the design calculations.
• If proprietary system of formwork (L&T Formwork system, Doka formwork system, etc.) is used
to overcome practical issues due to site / structure to be cast, detailed drawing and design shall be
submitted to the engineer for approval.
Finishing of Formwork-
• Finishing shall conform to IRS: CBC (CL. 6.2.4 and CL. 6.2.5) (Refer Appendix-1).
Cleaning and Treatment of Forms-
• Cleaning and treatment of forms shall conform to IRS: CBC (CL. 6.3) (Refer Appendix-1).
Specialized Formwork (Slip Form)-
• For slip form, the rate of slipping the formwork shall be designed for each individual case
considering various parameters including the grade of concrete, concrete strength, concrete
temperature, ambient temperature and concrete admixtures.
• In order to verify the time and sequence of striking or removal of specialized formwork, routine
field tests for the consistency of concrete and strength development are mandatory and shall be
carried out before adoption.
Stripping and Removal of Formwork-
• Stripping time shall confirm to provisions laid down in IRS: CBC (CL. 6.4) (Refer Appendix-1).

• The scheme for removal of formwork (i.e., de-shuttering and decentring) shall be planned in
advance.
• The formwork shall be removed in such a manner that does not cause any damage to concrete.
Centring shall be gradually and uniformly lowered in such a manner that it permits the concrete to
take stresses due to its own weight uniformly and gradually to avoid any shock or vibration.
• Where there are re-entrant angles in the concrete sections, the formwork shall be removed at these
sections as soon as possible after the concrete has set to avoid cracking due to shrinkage of
concrete.

9. Quality Control, Inspection and Testing


Reinforcement Steel-
• Physical tests shall conform to IS 1387, IS 1599, IS 1608 and IS 1786. Independent test on quality
of steel from each lot (each heat number) shall be carried out as per IRS: CBC (CL. 4.5.2)
(Refer Appendix-2).
• Sampling frequency and testing shall be as per Appendix-2 of this Work Method Statement.
• Reinforcement steel shall be inspected prior to the commencement of works and assembly on Site.
Defective, brittle or burnt bar shall be discarded. Cracked ends of bars shall be cut out. All
reinforcement steel shall be free of loose small scales, rust and coats of paint, oil, mud.
• Quality control of mechanical joints shall be as per IS 16172.
Inspection of Form Work-
• Formwork shall be inspected, and “Formwork Assembly Inspection Record” shall be submitted
prior to concreting works.
• "Formwork Assembly Inspection Record" describes the results of verification of inspection results
of the formwork with design documents in which the shape and dimensions of the formwork, clear
cover to the outermost reinforcement, effective height are verified.

10. Environmental and Safety Requirements


• All the safety norms and precautions shall be taken as per site safety plan (HSE Plan) and
clearance shall be taken before commencing.
• Level staff shall be made of non-electricity conductive materials.
• If night work is to be carried out, the area shall be sufficiently provided with lighting.

• Check for the working condition of safety valves of air compressors, if compressed air is being
used for cleaning.
• Necessary safety signboards and precautionary boards shall be placed near working locations.
• Safety stewards shall conduct daily checking to ensure the working areas are safe.
• Daily pep-talks shall be conducted to all workers at site to create awareness and to follow
safe working practices.
• Drinking water, toilet and rest place shall be suitably located for the easy access to
workers. All the facilities shall be properly cleaned and maintained during the entire period
of operation.

11 Inspection and Testing Plan


Carry out all tests required as per Appendix –2.

12. Format & Checklist


Following Documents of Checklist shall be maintained during execution of Foundation works to
maintain the record as mentioned in Appendix -3.

At all intersections, reinforcing bars shall be securely bound together with 1.6mm dia mild steel wire in
accordance with IS:280 or with approved reinforcement clips. The free ends of the binding wire shall be
bent inwards. For aggressive environment, galvanized binding wire shall be used.

2. All steel fabrics shall be lapped two meshes unless otherwise shown on the drawing and securely bound to
the supporting bars with 1.6mm dia mild steel wire (IS:280) or approved reinforcement clips. The free ends
of the binding wire shall be bent inwards. Proper cutting pliers shall be used and the wire binding and tying
shall be done as tightly as possible.

3. Unless otherwise specified by the engineer, reinforcement shall be placed within the following tolerances:
(a) For over all depth 200 mm or less : ± 10mm
(b) For over all depth more than 200mm : ± 15mm

5 The cover shall, in no case, be reduced by more than onethird of specified cover or 5mm whichever is less.
Reinforcement
4. Spacers should be of such material and designs as will be durable, will not lead to the corrosion of
reinforcement and will not cause spalling of the concrete cover. Spacer block made from cement, sand and
small aggregates should match the mix proportion of the concrete as far as is practicable with a view to
being comparable in strength, durability and appearance. The use of the pieces of wood, tile or porous
material will not be allowed for this purpose.
