AFFIDAVIT OF CHANGE OF GRADE

I, ROMMEL F. SACEDA, single, Filipino and a resident of 6 Ponco St., San

Francisco Del Monte, Quezon City, after having been duly sworn to in accordance
with law, depose and say:

1. That I am a faculty of National University - College of Allied Health -

Department of Medical Technology;

2. That I previously taught General Pathology with Histopathologic and

Cytologic Techniques (Lecture and Laboratory) in the above-mentioned
university during the first semester of school year 2021-2022;

3. That I am executing this Affidavit due to the mistake in giving a grade of

R to those students (see attached list) who took the course last
academic year of 2021-2022;

4. That, again, the reason for the change of grade of the abovenamed
students is due to my inadvertence in giving them their correct grade.
