I, ELEAZAR L. CANDIDATO, SR. of legal age, Filipino, with permanent address at

Valdemoro St., Poblacion, Baleno, Masbate.

After being sworn in accordance with law, do hereby depose and state that:

1. Each of the document submitted satisfies the requirements for the New/Renewal of
Application for Permit/Recognition to Operate Pre-School/Elementary/Junior High
School/Senior High School under the K to 12 Program Implementation is an authentic
copy of the original, complete, and all statements and information provided therein, are
true and correct;

2. By executing this Certification of Authenticity and Veracity of Documents, I hereby

authorize the Quality Assurance Division, DepEd ROV to verify all the documents
submitted;

3. I am aware of and have undertaken the following responsibilities:

a. Carefully examined all of the requirements as required by DepEd Order No. 88, s.
2010, as amended by DepEd Order No. 11, s .2011, and
b. Acknowledged all conditions in the Manual of Regulations for the Private Schools
Operation and other related laws.
