PARENTS/GUARDIAN CONSENT AND WAIVER

TO WHOM IT MAY CONCERN:

The undersigned, being the parent/guardian of Prince Joshua D. Paloay, who is enrolled at
Valencia National High School of Grade 11 Section MAPAGMASID hereby state that.

1. I am giving my consent to my son/daughter to Participate in a Research project in

Practical Research 1, 2nd Semester, School year 2023-2024.

A. Research Title: The Ibabasuk Ritual of Talaandig Tribe in Lilingayon Valecia City

B. Purpose: Our study on the Ibabasuk Ritual of Talaandig Tribe aims to analyze
how the Talaandig people perform their ritual, specifically the Ibabasuk ritual.
The goal is to know the purpose and the benefits of the Ibabasuk ritual in the
lives of Talaandig people in Lilingayon Valencia City

C. Duration: April-May 2024

D. Location: Barangay Lilingayon

And I hereby consent thereto:

2. Having been appraised and adequately informed of the preceding, I hereby grant the
right to their teacher to do, with proper diligence, any legal act necessary to preserve
the well-being, safety, and security of my son/daughter and her companions during
the said activity.
3. Having been appraised and adequately informed of the foregoing, I hereby declare
that I will not hold the Teacher, School Management, the Division Office, or the
Department of Education for any incident that may occur beyond their control.
