Dear Miss/Ma'am/Sir,

I am writing this on behalf of the students in Grade 12-HUMSS Joyce we would like to ask for your
approval for our Outreach Program, which is served as our performance task for the Introduction on the
Disciplines and Ideas in the Applied Social Sciences (DIASS) subject. The outreach will be held at the
Purok BM 3rd road, Barangay Taculing, Bacolod City.

As part of our DIASS subject notably Social Work, we are strongly encouraged to perform an outreach
program. The purpose of our outreach program is to Establishing partnerships with local organizations
for resource-sharing. We believe that this initiative will have a positive impact on the community by
providing immediate assistance to the community through food and clothing donations, and we are
excited about the opportunity to work with your organization to make a difference.

The said activity will be on December 16, 2023 Saturday at 1:00 PM–5:00 PM.

While we recognize the importance of community engagement, we are also committed to delivering our
best performance at DIASS, We assure you that we will remain dedicated to both our academic
responsibilities and community involvement, and we appreciate your consideration of this request.
