Scenario On January 20, 2024, at 06:30hrs., at SDGM - 1267. While traveling to the job
site, the site supervisor was riding vehicle with four other workers. Inside the
vehicle, several personnel installed various obstacles that limited visibility
while driving. The foggy season causes a lack of sight when turning the
automobile, which leads to an accident with a different car that was parked
on the side of the road. One of the workers who was hurt was among the
trapped employees, along with the driver. As indicated in the JMP Sheet, the
site supervisor immediately dialed on the emergency number for SDGM region,
traffic department and the incident commander. The IVMS Coordinator
immediately determines the location and update the IC with the location of
the vehicle. This incident has also been reported to the SAPMT Safety Advisor,
Project Manager, and event Commander. In an emergency, the incident
commander acts swiftly. After a short while, the emergency medical team
along with search and rescue team came, and they helped the trapped
workers escape from the wrecked cars. The ERT medical team administered
first aid to the wounded workers right away, and then all of the workers were
sent to the hospital for additional medical care and exams.
Note: Planning and review of all drills should eliminate all risk of actual injury or incident. All communications
written and verbal, should start and end with “This is a Drill, This is a Drill, This is a Drill”
