Foreword
Nursing is an art and science. A nursing student is expected to master both theory and practical
skills. The skills that are mastered to date must continuously be practiced to remain proficient.
This logbook forms part of the requirements for successful completion of the general nursing
programme. It provides evidence of skills acquisition for registration with Lesotho Nursing
Council. It is the student’s responsibility to keep the logbook safe, as it will be needed for the
each year of training in the programme. The student shall remind the assessor/preceptor to
complete the logbook. After the student clinical placement, the logbook shall be presented to the
clinical supervisor. The assessor/preceptor shall supervise and ensure that students achieve the
skill required for proficiency and registration. The assessor/preceptor will also comment and sign
the appropriate sections of the logbook.

developing this log book, especially Ms. Lastina Lwatula and Ms. Rosemary Kamunya for their
encouragement and support, as well as Pre-service Education Advisor, Ms. Maleshoane
Monethi-Seeiso, Programme Assistant, Ms. Melida Lepheane. We would also like to thankthe
management of nursing education institutions namely National University of Lesotho (NUL),
National Health Training College (NHTC), Roma College of Nursing, Scott School of Nursing,
Paray School of Nursing and Maluti Adventist College of Nursing for releasing the Nurse
Educators for participating in this endeavor. Lastly many thanks to all Nurse Educators for their
contributions and commitment.

Purpose of the Logbook
The Student Practicum Logbook is a document that will be:
1. Presented to the Clinical Preceptor/Assessor for recording activities each time the student
observes, assists or performs a procedure.
2. It will be incumbent upon each student to present the logbook to the clinical
preceptor/assessor each time a student observes, assists or performs a procedure.
3. The Logbook should be completely filled in and signed by both student and Clinical
Preceptor/Assessor prior to student taking the final examination.
4. This logbook will determine how often each procedure will be observed, assisted or
performed.
5. This logbook will determine how often the Clinical Supervisor will check the log book.
The aim of the logbook is to keep a record of student’s learning experiences and practice toward
attainment of competencies and skills required to meet prescribed registered nurse scope of
practice.
Specific Objectives
By the end of the clinical placement, the student should be able to:
1. Apply basic biomedical sciences, applied medical sciences, research, ethics and professionalism
in provision of care;
2. Attain skills in practical procedures, diagnosis, therapeutics and management of patients in
various clinical settings;
3. Demonstrate attitude on awareness of own limits and willingness to refer clients at appropriate
times;
4. Demonstrate ethical practice, empathy, respect for high quality care; and
5. Exhibit a desire for continued professional development.

Practice Assessment Guidelines
1. When a student has performed a procedure, she or he will ask the assessor/preceptor to
comment, sign and date the logbook.
2. The student is allowed to have two (2) observations on a procedure, one in the skill’s lab
on a manikin and the other on the real patient in clinical area.
3. The learner is to do three (3) return demonstrations on a procedure, in which the first two
are to be signed either by assessor/preceptor and the last one by assessor (Nurse
Educator).
4. At the end of each placement the logbook should be submitted to the clinical supervisor
for review and as deemed necessary and on completion of the programme the logbook
will remain the student’s personel file.
5. The student is expected to perform all prescribed procedures in the relevant year.
6. If a skill cannot be obtained during current clinical placement, the student will initiate a
mutually agreeable plan with the assessor/preceptor for return demonstration.
