PARTICIPANT’S WAIVER AND RELEASE:

I, ____________________________________, a minor, do hereby declare as follows:
1. I have asked permission from my parents and they have permitted me to join in Tawid Barangay
Anniversary Fun Run 2023 that will be hold on the 31th day of December 2023 in Tawid, Maasin
City, and that my parents have also understood that there is a risk in my participation in the
mentioned event.
2. I know that competing in Tawid Barangay Anniversary Fun Run 2023 is a potentially strenuous
running activity. Thus, I know in myself that I am healthy and capable to join.
3. I assume full and complete responsibility for any injury or accident which may occur while I am
participating in the event.
4. I agree and abide by all race rules and directions mandated by the Event Organizers.
5. I also acknowledge that there is a risk of injury from various causes including dehydration,
accidents, overexertion, weather conditions, and other causes.
6. I waiver all claims of liability or damage against any, individual, group, the host of the event,
and the organizers.
7. I will obey restrictions from the organizers for the sake of safety and welfare.
8. I have attached my Full name and signature of my parent/guardian below.
