PROCEDURE RESPONSIBILITY
AUTHORITY:-

Only an appointed and authorized person is permitted to drive an A.D.T. Supervisor

P.P.E. REQUIRED:-
Supervisor
 Hard hat – worn when outside of the cab
 PVC gloves – worn when handling material
 Steelcap/Safety Boots gumboots – worn continuously
 Earplugs – worn whilst operational
 Reflective vest – worn when outside of the cab
 Dust mask – worn in dusty conditions.
TOOLS AND EQUIPMENT REQUIRED:-

 Checklist Supervisor
 Lock-out and key
 Two stop blocks
 Two hazard triangles
 Fire extinguisher

TASK 1 PRE –USE INSPECTION:-

 A daily pre-use inspection as per checklist must be carried out at A.D.T. Driver &
the start of each shift, on completion to be signed off by all relevant Mechanic
personnel.

PROCEDURE RESPONSIBILITY
1. CONTINUED:-
 When inspecting the engine ensure the engine hood retaining bar
has been securely installed.
 Articulating locking bar to be securely installed.
 The A.D.T. must be isolated, locked-out, the park brake applied and
a stop block placed on either side of a wheel.
 Be cautious of moving Machinery/Vehicles/Haulers in the works
area.
 Missing wheel stud nuts must be replaced, ensure all wheel stud
nuts are securely positioned.
 Missing wheel studs are to be replaced, ensure all are in position.
 Damaged/missing load bin tipping restriction chain to be replaced,
ensure correct length.
 Damaged/missing steps/handrails are to be repaired/replaced.
 Mounting steps are to be kept clean.
 Tyres are to be inflated according to O.E.M. specifications.
 Worn/cracked sidewall tyres are to be replaced.
 Be cautious of hot engine parts.
 When opening a hot radiator cap a large damp cloth must be used,
maintain firm pressure on the radiator cap and gradually release the
pressure from the system, the engine must be left running when
topping up a hot engine with radiator water.
 Top up all low fluid levels.
 The windscreen/windows/rear-view mirrors must not be damaged
and must be kept clean.
 Moving parts must be guarded.

TASK 2 OPERATIONAL PRE-USE INSPECTION:-
 Ensure the articulating locking bar has been removed.
A.D.T. Driver &
 Maintain sure handgrip, sure footing and face the cab when
embarking and disembarking. Mechanic
 Worn fellow workers before starting.
 Ensure the A.D.T. is engaged in neutral and the park brake is applied
before starting.
 Check if PDS is operational (where fitted)
- On start-up the hooter must sound.
- On approaching a vehicle/mobile machine or pedestrian a voice
command will caution you to be careful, travel at a crawl and observe
the surroundings until clear.
- The distance from a machine/vehicle or pedestrian will be displayed
on a screen.
 Do not rev the engine.
 Leaking pipes/hoses/fittings must be repaired/replaced.
 The following must all be operational:-
- Wipers
- Head/Tailgate lights
- Strobe light
- Reverse siren
- Hooter
- Indicators.
 Worn wiper blades must be replaced.
 Ensure the retarder is operational.
 Faulty foot brake must be repaired.
 Faulty park brake must be repaired.
 If an ‘A’ CLASS HAZARD is noted during either inspection or during
the course of the shift, the A.D.T. must be isolated and locked-out until the
HAZARD has been repaired.

TASK 3 PROCEED TO THE WORKS AREA AND TRAVEL
WITH LOAD:-

 A seat belt must be worn at all times whilst operational. A.D.T. Driver
 Only 1 passenger is permitted, to be seated in the seat that has been
provided and must be belted in.
 Whilst operational:-
- Talking on a cell phone is prohibited
- Headlights must be switched on.
 Wait for normal operating temperatures to prevail before pulling-off.
 Adhere to the National Road Ordinance.
 Adhere to the main Mine Road speed limit of 35 km/h
 On approaching the works area reduce speed.
 Ensure the F.E.L./Excavator Operator is aware of your presence;
carry out the instructions given by the F.E.L./Excavator Operator.
 Ensure the correct product is loaded.
 The loading area must be level.
 Whilst operational the heat gauge must be monitored, on noting
excessive engine temperature, the A.D.T. must be isolated and
locked-out.
 Before negotiating an incline/decline engage in a lower gear and a
brake teat must be done.
 When negotiating a decline with a load the retarder must be activated
until the decline has been negotiated.
 Keep clear of the highwall.
 Keep clear of embankment edges.
 Only one Vehicle/Machine is allowed on a ramp at a time.
 Loaded Haulers/Dumpers have the right of way.
 The retarder must be engaged in wet weather, hauling to be stopped
in extreme wet weather.
 Avoid cambered areas in wet weather.

TASK 3 CONTINUED:-
 Reversing up an embankment must be done in a straight line and not
at an angle.
 On noting an approaching Vehicle/Machine headlights must be
dimmed.
 Ensure that the 3m Radius Clearance Rule is observed, if not the
A.D.T. must be stopped until compliance is adhered to.

TASK 4 LOADING:-
 Ensure the F.E.L./Excavator Operator has noted your arrival and
A.D.T. Driver
adhere to his instructions.
 Ensure that the F.E.L./Excavator has sufficient reach to load
efficiently into the load bin, without damaging the load bin structure.
 Whilst loading is in progress the Driver is not permitted to get out of
his/her cab.
 Prior to loading a new product it must first be ensured that all material
from the previous product loaded has been cleared form the load bin,
in order to prevent contamination.
 The A.D.T. must not be overloaded.
 The load must be evenly distributed in the load bin area.
 The loading area must be level.
 Dumper articulation must be aligned, prior to loading.

TASK 5 TIPPING:-

 Reduce speed on approaching the tipping area. A.D.T. Driver


 Ensure the load is tipped at the correct product.
 Do not tip directly under overhead power lines.
 Uninvolved personnel are to vacate the works area.

TASK 5 CONTINUED:-

 Dumper articulation must be aligned. A.D.T. Driver


 The tipping area must be level.
 Tipping must not be done in extremely windy conditions.
 On tipping completion, travel forward at a crawl, until the load bin
tailgate has cleared the tipped material, the load bin must then be
fully retracted.

TASK 6 BREAKDOWN AND MAINTENANCE PROCEDURE:-

 If a breakdown occurs en route, the A.D.T. must be parked as far as A.D.T. Driver &
possible on the left hand side.
Mechanic
 The A.D.T must be isolated and locked-out, the park brake applied
and a stop block placed on either side of a wheel.
 A hazard triangle must be installed at a distance of 20m on either
side of the A.D.T. respectively.
 Be cautious of other moving Machinery/Vehicles in the area.
 Be cautious of hot engine parts.
 Be cautious of hot engine parts.
 If radiator water is to be topped-up, refer to Task 1.
 Work may not be done under a raised load bin unless
support blocks have been installed.
 If jump-starting is required to be done the following steps are to be
adhered to:-
- The vehicle/machine housing the helper battery must be isolated
before attempting to connect the jumper cables.
- Uninvolved personnel are to vacate the works area.
- Only a helper battery of equal capacity is to be used.

6.CONTINUED:-
- Only heavy duty jumper cables are permitted to be used.
- Jumper cable clamps must be in good condition and form an effective
circuit.
- Do not let jumper cable clamps touch each other.
- When handling the jumper cable clamps, handle only on the rubber
grips on the clamp handles.
- Ensure correct polarity + to + and – to − .
- + Is always red.
- On connecting the jumper cables the Vehicle/Machine housing the
helper battery must be allowed to run for a few minutes before
attempting to start the A.D.T.
 Oil must be drained into a container and disposed of in a manner
prescribed by the Mine Manager.
 When refueling:-
- The A.D.T. must be switched odd and the park brake must be
applied.
- No smoking symbolic sign to be displayed.
- No naked or open flame symbolic sign to be displayed.
- No cell phones symbolic sign to be displayed.
 Prior to inflating a wheel, a certified wheel cage must first be securely
installed over the wheel.
 If the brakes have been worked on:-
- A brake test must first be carried out on a flat area free of
pedestrians, machines and vehicles.
- Thereafter a brake test must be carried out again on the brake test
ramp.

TASK 7 CHANGING A WHEEL/JACKING:-

 Only a compatible weight jack is permitted to be used. A.D.T. Driver &


 Uninvolved persons must vacate the works area. Mechanic
 The jack must be placed on a firm solid foundation.
 The jack lifting piece must be fully inserted under the portion to be
lifted.
 Only a standard jack activating lever/crank is permitted to be used to
activate the jack.
 Ensure freedom of hand movement prior to jacking.
 Be cautious of vehicles/machinery in the works area.
 Wheel nuts must be loosened diagonally across from each other.
 Wheel nuts must be tensioned diagonally across from each other.
 On removing the jack the wheel nuts must be re- tensioned.

TASK 8 END OF SHIFT PROCEDURE:-

 Park in the designated parking area. A.D.T. Driver


 The park brake must be applied.
 The Dumper must be isolated and locked-out.
 A stop block must be fitted on either side of a wheel.

PRECAUTIONS TO TAKE:-
 Always be aware of the HAZARDS mentioned in this procedure.
 As an A.D.T. Driver you are always exposed to these HAZARDS, if
A.D.T. Driver
overlooked could result in PERSONAL INJURY, PROPERTY
DAMAGE and/or a PRODUCTION LOSS.

AUTHORITY:-

 As per Section 23 of the Mine Health and Safety Act you have the A.D.T. Driver
RIGHT to REFUSE to work in seriously dangerous working
conditions and with substandard equipment and/or machinery.

RESPONSIBILITY:-

 As per Section 22 of the Mine Health and Safety Act you are A.D.T. Driver
responsible for your own SAFETY and for the SAFETY of your fellow
workmen.

PROTECTION OF THE SAFETY OF FEMALE EMPLOYEES:-

 All female employees are to carry a whistle and a can of pepper All Employees
spray.
 All employees are to adhere to the Procedure for the
PROTECTION OF THE SAFETY OF FEMALE EMPLOYEES
