To whom it may concern:

I am writing this letter to formally explain what happened regarding the incident I was involved. I got sick
on November 28, 2023, and I was not able to seek medical consultation and what I did was only self-
medication. By the second week of December, my HRI, Manalo kept on reminding us to pass the LOA as
well as the medical certificate.

By that time, I went to my friend personally and sought her help once again to help me with the medical
certificate. To explain what happened, year 2022-2023 I was working in a BPO industry (call center) they
provided us a HMO insurance and I was not aware of the full perks and advantages of using this card, my
friend KC helped me to link my card to the Doctor Anywhere App. To be honest my friend used to be my
“proxy” whenever I needed a medical certificate for my work before as I did not have enough time to do
consultations since I am full-time working in Concentrix and a student as well, she used to do
consultations for me both teleconsultations and personal consultations, for the ID card, I did have a TIN
ID which has her picture and this is what she used for consultation before. Since then, the medical
certificates that she handed me have all been accepted by the company, hence I know it is legitimate
papers. Since I did not seek consultations last November, on December 13, 2024, I went directly to her
house to ask her a favor to be my proxy again and to do consultation for me since I have night duty, since
she understands my situation that time, she responded “yes” however upon checking the app there was
already a professional fee ranges P600 above. I sincerely asked her if she could lend me P400 and I would
pay her after 5 days since all the money I had that day was only P300, however, she also did not have
extra money, she told me she would look on Facebook pages for a much cheaper fee then I said “sige
ikaw na bahala basta kailangan ko na yan bukas”, I went to my night duty and then by tomorrow that day
they gave me the letter. Since I was confident enough that the medical certificate she gave was
legitimate and real, I passed it in the clinic, the University Physician even asked me if I really had a video
consultation with a doctor, and I said “yes” since all I know is my friend had a real teleconsultation with a
real Doctor. When Doc Isidro said that the medical certificate was dated February, it draws too much
confusion to me since I knew that my friend never given me a falsified document even before. I went
straight to her to ask what was that, then she admitted that upon checking on Facebook pages for
cheaper options she found a P250 fee however the physician does not do backdated certificates, since I
was not around that time, she decided to edit the previous medical certificate on the app since I told her
that I really need the medical certificate by tomorrow that day. I do not have any intention to falsify any
document. I totally admit that arranging someone to do consultations for my convenience is totally
wrong, I admit and I am sincerely apologizing for that.
