Dear Mr. Anasco,

I am writing this to seek consideration for my request to have a 1-month leave from my
teaching duties at Mambugan National High School. It is for this reason that I personally
need a month-long break in order to have the best medical assistance due to my health
complications.

In regard to the tasks and responsibilities I have to bestow on my substitute teacher, I

have already arranged and created the lesson plans and activities for my one-month-
long leave.

I fully comprehend that my job as a mathematics teacher in MbNHS is important and

may affect the circumstances in the school, especially in my classroom. Henceforth, I
have made my lesson plans comprehensible and cohesive, as well as the materials
needed by the substitute teacher, as much as possible in order to make the classroom
activities run smoothly.

I look forward to the approval of this request of mine on the upcoming dates of October
5, 2026, to November 1, 2026, with the intention of returning to my job on November 2,
2026. It is entrusted that my pending teacher responsibilities are all done before my
supposed leave.

In this letter, the documents needed to support my request for leave are attached. I am
