COMITMMENT LETTER

As an employee of Informatics, I understand my role and the importance of meeting the objectives of my role to the success of the Company. I also
understand the purpose of this goal setting documentation in the Company’s Performance Management System. Moreover, I am aware of the
consequences of my failure to meet the objectives of my role. With this, I agree to the following:

1. To create plans and work diligently to meet my performance targets as detailed above.
2. To communicate with my Immediate Head my progress by submitting a monthly report detailing my accomplishments and any challenges that
may affect my performance.
3. To seek performance feedback from my Immediate Head and my colleagues to ensure that I am on track.
4. To ask for support (resources, training, etc) whenever necessary.
5. To work on additional tasks that may be given beyond the performance targets that are listed above.
6. To ensure that I meet at least 80% of the performance targets listed above.
7. To abide by the rules of the Company including my Employment Contract and the Company’s Code of Behavior relative to failure to meet
performance targets.
