JUDICIAL AFFIDAVIT

I,____________________ ,of legal age, single and a resident of Ayala, Zamboanga

City, after having been sworn to in accordance with law, do hereby state as follows:

PREPARATORY STATEMENT

I am answering the questions propounded by Atty._______________, of

____________Law Office fully conscious, that I am under oath, and that I may face criminal
liability for false testimony of perjury;

This Judicial Affidavit is written in English, a language known and understood by me,
and the examination and execution thereof were done at the same office located
at____________;and

This Judicial Affidavit shall serve as my testimony in this case.

Q1: Do you swear to tell the truth and nothing but the truth?
A1: Yes.

Q1: Please state your name and other personal circumstances.

A1: I am _________,45 years old, Filipino, married, and a resident of Ayala, Zamboanga City
where I serve as a barangay kagawad in Barangay Ayala, Zamboanga City.

Q2: What is your interest in this case?

A2: I am here to give my testimony as a witness in favor of Jerwel Casumpa.

Q3: Do you know the accused Jerwel Casumpa?

A3: Yes. I personally know Jerwel Casumpa having been a resident of Barangay Ayala for
several years now and also being one of his regular customers who patronize the milk he
delivers.

Q4: What can you say about the accused?

A4: He is a good man who always made himself available to help others. Often times, he
volunteers to help us in our Barangay programs specially during pandemic.

Q5: Can you tell me more about him?

A5: Yes. He is friendly to all of his customers and I remember once he told me that there are
few of his customers who incurred loan from him and he was shy to ask for payment despite his
low income.

Q6: Was there a chance that the accused figured into a fight with someone from the same
barangay?
A6: There was none. I normally saw him helping his parents or delivering milk to his
customers. He rarely makes friends in the neighborhood nor had any arguments with others.

Q7: Is there anything you want to add?

A7: None attorney.

I execute this Affidavit to attest to the truth of the foregoing facts based on personal
knowledge and authentic records and for whatever legal purpose it may serve.
