INTRODUCTION

The international collaboration between the Federation University of


Australia and the Universitas Airlangga in Indonesia has been going well
over the years through joint research and other collaborative activities.
To improve this cooperative relationship, the two Universities will carry
out an activity called the “New Colombo Plan” Program funded by Grant
from Federation University, Australia and led by Associate Professor
Muhammad Aziz Rahman MBBS, MPH, CertGTC, GCHECTL, PhD as
Program Coordinator. About 21 foreign students and 3 staffs (co-
lecturers) from the Federation University of Australia in International
Business and Nursing will participate in the program in Indonesia,
arranged by Universitas Airlangga. This activity will be held with the
hope that Australian students will be able to interact with students and
faculty members from Universitas Airlangga, as well as the surrounding
community in the East Java region to have a better understanding of the
cultural context of handling public health, especially in terms of health
promotion and disease prevention efforts in Indonesia. They will also be
able to share their experiences in Australian healthcare. Special visits
will be arranged for Australian students to interact with the Mayor of
Surabaya, who has taken a number of public health initiatives to
improve the health and well-being of the people there and has been
rewarded nationally and internationally. In addition, this event will be
an opportunity to visit other parts of the East Java region, such as
Pasuruan and Lamongan.

OBJECTIVES

Day 1 Exploring Universitas Airlangga

Health Promoting University (HPU) is a systematic and comprehensive


effort in realizing higher education as an institution that integrates
health and health promotion efforts as part of the higher education
culture which is reflected through daily operational activities,
administrative management, and academic mandates.

Universitas Airlangga has been an active member of AUN-HPN since


2017 and declare internationally as the centre of health and wellness in
the first conference of AUN HPN in Philippine in 2019. Universitas
Airlangga as one of the higher education institutions are committed to
creating an organizational environment and culture that can improve
the health and well-being status of the entire academic community.

AHPC is a multidisciplinary institution that is intended to improve public


health status internally at Airlangga University and external
communities around Airlangga University by working and networking
with all disciplines to improve public health status through teaching,
research and community service.

There are many efforts that has been done by AHPC to support health
promoting university through advocating healthy public policy,
reorienting health services in the campus, improving skills of individuals,
strengthening community action, and creating supportive environment.
One of the faculties in Universitas Airlangga that also aimed to improve
the public health is Faculty of Public Health. Therefore, it is important to
understand many efforts that has been done to promote health
promoting university at Universitas Airlangga

Day 2 How to Implement Health Promotion in Hospitals?

According to Permenkes No. 44 of 2018 concerning Implementation of


Hospital Health Promotion (PKRS) is a process of empowering patients,
patient families, hospital human resources, hospital visitors, and the
community around the hospital to actively participate in the care
process to support behavior and environmental changes and maintain
and improve health towards achieving optimal health status. The
purpose of achieving optimal health status is not only aimed at patients
or people who are sick. The word "optimal" means that everyone has
the opportunity to improve their health status. Everyone is of course in
a position of different health degrees, therefore health promotion
activities and objectives must be adapted to the health status of each
target. The aim of health promotion for patients or sick people is to
support optimal treatment from illness to health. PKRS activities are
indeed carried out by the PKRS team, but in the end it must be able to
make each human resource in the hospital become a health promoter
in their respective parts. It can be said that PKRS activities are team
activities involving cross-professions and sections where each section
must coordinate well and complement each other. The aim of this cross-
professional collaboration and division is to produce educative,
applicable and reliable health promotion materials which can prevent
misinformation from circulating in the community.

What is the function of PKRS? The PKRS function is a hospital effort to


improve the ability of patients, clients and community groups. So that
patients can be independent in accelerating their healing and
rehabilitation.

Day 3 Learning about Community Empowerment

The following are some of implementation Community Empowerment


in Surabaya :

a. Puskesmas (Community Health Centre) are government-mandated


community health clinics located across Indonesia. They are
overseen by the Indonesian Ministry of Health and provide
healthcare for the population on sub-district level. Community and
preventive health programs formed another component of
Indonesia's health system.

Tanah Kalikedinding community health center (Puskesmas Takal) is


a health center that is included in the Tanah Kalikendinding
subdistrict area, Kenjeran district, North Surabaya.Puskesmas
Kalikedinding has a working area of ± 350.54 Km2 consisting 12 RWs
(Community Units), 144 RTs (Neighborhood Units), a population of ±
58,301 people (29,142 men & 29,159 women). The vision and
mission of Puskesmas Takal are to improve the service quality
management system, increase the professionalism of human
resources, provide comprehensive and integrated health services,
and improve services for UKM (public health efforts) and UKP
(individual health efforts). Apart from that, Puskesmas Takal also has
UKBM (community-based health efforts), namely Posyandu
(integrated service post) for toddlers, Posyandu for the elderly,
Posbindu (integrated service post for communicable diseases),
Posyandu for teenagers, UKK (occupational health) Post, Poskestren,
and SBH (Saka Bakti Husada). Puskesmas Takal also provides general
inpatient, childbirth inpatient, and emergency room services.

b. Pesantren is an Islamic religious educational institution that plays an


important role in the development of human resources (Indonesia,
2013). Pesantren is a place where the risk of disease transmission is
pretty high because pesantren is a place where students gather in
one room, such as sleeping, eating, learning rooms and using shared
bathrooms. Conditions in the Islamic boarding school environment
still require attention from related parties, especially covering three
aspects, namely access to health services, Clean and Healthy Living
Behavior (PHBS), and environmental health (Hulaila et al., 2021).
Some diseases that are closely related to the Islamic boarding school
environment. Surabaya has 25 Pesantren with 1689 students living
in dormitories and 757 students not settling (PDPP, 2019). In 2018,
East Java Province included in ten Provinces with the highest
percentage prevalence of ARI is 9,5% where the incidence of ARI in
East Java is also contributed by the Islamic boarding school
(Istifaiyah, Adriansyah and Handayani, 2019).

The Pesantren Health Post (Poskestren) is a form of Community-


based Health Efforts (UKBM) in the Islamic boarding school
environment, with the principle of, by, and for the residents of the
Islamic boarding school, which prioritizes promotive and preventive
services, without neglecting the curative and rehabilitative aspects
carried out under the guidance of the local Puskesmas.

c. Healthcare Center for toddlers and elderly in Tanah Kalikedinding


Primary Health Center
Healthcare Center / Posyandu is a mix of the Bahasa Indonesian
words pos, pelayanan, and terpadu, together meaning “integrated
service post”. As the name implies, it is a community activity which
‘integrates’ essential health services for mothers and children living
in rural areas. Such services cover areas of maternal and child health
including family planning, nutrition, immunization, and disease
control. Posyandu as an initiative was created by the government in
the mid-1980s in an effort to reduce maternal and child mortality as
well as pregnancy rates. A few years later, the government created
the village midwife program which involves a midwife being trained
or placed in each village in the country, which then became
integrated with Posyandu.

Posyandu has many program branches, one of which is Posyandu


balita (healthcare Center for toddlers) and Posyandu lansia
(healthcare center for elderly). Posyandu Balita is a service to
toddlers and children by weighing so their growth and development
can be monitored.

By joining Posyandu Balita, mothers may get benefits including


children health services, immunization, supplementary feeding, and
counseling about health. Posyandu Balita in Puskesmas Takal are
supported by pediatric clinic and maternity clinic. Puskesmas Takal
has 14 Posyandu Balita spread throughout its work areas.

Posyandu Lansia or Posyandu for the elderly is a health service that


not only provides services to curative efforts, but also focuses on
promotive and preventive efforts. Puskesmas Takal has 3 Posyandu
Lansia. Posyandu lansia in Puskesmas Takal provides health services
needed by the elderly, counseling, elderly exercises, supplementary
feeding programs, entrepreneurship, and health checks for the
erlderly. These programs are carried out regularly and scheduled.

d. Other community empowerment is hydroponic in Medokan Ayu.

This program about community empowerment efforts and


improving the quality of life of the community, the hydroponics
program carried out in RW 12, Medokan Ayu Subdistrict, Rungkut
District, provides an opportunity for the community to grow and sell
hydroponic vegetables while creating a green city environment.
Products of the hydroponics program become an MSME (Micro,
Small, and Medium Enterprise) center for its citizens which is named
"Hidroponik Medayu Utara" (Hydroponic of the North Medayu) or
"HidroponikMU".

In the MSME forum, all members perform various kinds of business-


based environmental conservation activities. Such as waste bank
management, collection of waste cooking oil, and utilization of
household waste turn into eco enzymes whose its water will be filtered
and sold. All programs made are efforts to improve food security, the
village environment, reduce household waste with the ultimate goal is
to reduce the concentration of air pollution in Surabaya.

Day 4 Looking into the History of Surabaya

The tour will take you to places where the stories of this great and heroic
city of Surabaya started. The history of this amazing maritime city spans
from the 14th century to the 20th, from classic to the independence
stories. The first place to visit is an ancient statue of Joko Dolog, the
embodiment of King Kertanegara (Singasari Kingdom 1222-1292). The
statue was brought from its original place in Bejijong, an ancient village
about 50 km to the south of the city to Surabaya during the brief period
of the British Administration of the Dutch East Indies at the beginning of
the 19th century. It was done by Baron van Salis, the Resident of
Surabaya in 1811.

From the statue site, the tour will move to an area called Pandean-
Peneleh where there is an amazing archaeological object ever found in
the inner city of Surabaya. It is in the form of a well, made of terracotta,
or clay. It is the well that had been there before 1430 AD. It is the
evidence of ancient civilization of Surabaya, especially Kampong
Peneleh. especially Kampong Peneleh.

Peneleh used to be one of the few first residences of Europeans during


the Dutch colonial time. One very interesting fact that not many people,
Soekarno, the first president, was born here in this area.

From Peneleh, then the tour moves north to the place where one of the
nine Islamic Saints (Wali Songo) in Java started to introduce Islam to the
local Hindu people at that time. It is called Kampong Ampel. As this was
the center of the Islamic influence in Surabaya at that time, Arab people
who later arrived in the port city, decided to live around the masjid
(Grand Mosque of Ampel).

From Peneleh, we will continue to Kota Tua/Old City (China Town and
Dutch Town) and the Red Bridge (Jembatan Merah). It is the town which
the Dutch created to imitate European towns. This area was practically
the original city center from the 17th to the 19 centuries. The Red Bridge
was famous for being one of the fiercest battlegrounds during the
famous Battle of Surabaya which took place just few months after the
independence of Indonesia. A famous incidence was undoubtedly when
British Brigadier Mallaby was killed near the bridge.

The last stop is definitely the Hero Monument, or Tugu Pahlawan, where
our first president, Soekarno, symbolized his hopes in the form of a
quintessential monument, as it did only represent the history of the
young nation that time but also provided the future paths of the nation
based on Soekarno’s idealism.

Day 5 Cultural Diversity

Indonesia can be categorized as one of the biggest countries in the


world. Due to these facts, Indonesia is well-known for its cultural
richness, which made Indonesia known as a multi-cultural country.
There are lots of cultural heritage, such as traditional dances and songs.

introducing these cultures to our communities and people around us.

The term "gembelengan" in the lyrics of "Gundul-Gundul Pacul"


refers to a leader who is not given a crown. The goal is to work for
the well-being of its people. However, in general, the leader will
adopt an arrogant or "gembelengan" demeanor. Based on
Javanese language, “Pacul” is an abbreviation of “Papat Kang Ucul”
(four missing things) :

- Eyes used to see people's or society's problems.


- Ears used to hear advice.
- The nose, which is used to detect the presence of justice.
- The mouth was used to uphold the truth.

The lyrics "nyunggi nyunggi wakul kul, gembelengan" which means


carrying a basket over one's head, becomes a symbol that a leader
must uphold the mandate of the people, but not with an arrogant
attitude (gembelengan).

The lyrics "wakul ngglimpang segane dadi sak ratan" mean that the
basket has been turned over and the rice has spilled onto the
ground. The meaning of "toppled basket" is a symbol of the
people's mandate that fell due to the messenger's arrogant
attitude. The phrase "segane dadi sak ratan" then represents that
the results obtained are messy and wasted, or that they can no
longer be eaten. It can also be interpreted as no longer being
beneficial to society.

b. Group 2 : Sajojo
We will invite the second group of participants to listen and learn
about one of well known folk song in Indonesia, The Sajojo Song. The
Sajojo song is a folk song from Papua that tells the story of a beautiful
woman who comes from a village. This woman is loved by her
parents and men in her village. This beautiful girl is called ‘kembang
desa’ and at the same time, she made men fall in love and wanted
to ask her for a walk. This song is also used as a dance
accompaniment with the same name-the Sajojo Dance. Sajojo dance
is a traditional dance from Papua. This dance has quite energetic
movement, full of joy and enthusiasm. Sajojo dance has the meaning
of the spirit of togetherness and describes joy among Papua people.
This can be seen from the expressions of the Sajojo dancers and the
movements that contain a lot of compactness and rhythm. The
Sajojo dance also describes the mutual cooperation of people living
in Papua. This dance is generally performed at traditional
ceremonies or to welcome tourists.

c. Group 3 : Rasa Sayange


Rasa Sayange (Eng: Loving Feeling) is an Indonesian folk song from
Maluku (east Indonesian archipelago comprising 2 provinces,
Maluku and North Maluku). This song uses Pantun, a poetic form
used to express intricate ideas and emotions.

There's many versions of the lyrics since it's a folk song. The lyrics
can be composed or adjusted depending on the message the singer
wants to convey. The massage can be about romance, joke, or even
life lesson. This song can be sung at social gatherings or even as
nursery rhymes, which it's represents a community and can be sung
or played by people who may or may not be trained musicians, using
the instruments available to them.

As a tradition, Rasa Sayange is a massage to convey affection, love.


This song also means compassion to the community since it passes
from generation to generation. And refers to the last verse of this
song, this song also conveys a prayer and hope for health and long
life.

Participants will be divided into three groups, and each group will
explain the selected traditional song and introduce them to their
teammates. All participants will be given 20 minutes to learn the
song and make a simple choreography. After the discussion session,
each group has to perform the result.

Day 6 Discovering Indonesian Traditional Medicine

Traditional medicine has been practiced in the world for centuries. Traditional
medicine refers to health practices, approaches, knowledge and beliefs
incorporating plant, animal and mineral based medicines, spiritual therapies,
manual techniques and exercises, applied singularly or in combination to
treat, diagnose and prevent illnesses or maintain well-being. Passed along
from generations to generations, some of the traditional treatment on some
region has gaining more popularity and is readily being incorporated into
modern day health care, not only in the developing world but also in the
developed countries.

Traditional Massage is commonly an ancient healing system that combines


broad and targeted stimulation and manipulation of energy inside the body.
It is believed as combination of art, science and culture. And as a non-
medicinal remedy and manual therapy, it involves bodily manipulation in
which the practitioner helps rebalance patient's body, energy and structure
or posture of human body. The Indonesian Traditional massage, namely
Javanese Massage, Balinese Massage or Borneo massage, whereas the part of
cultural hereditary inheritance, thus believed to enhance health, healing
illness, as well as maintaining beauty and fineness. The traditional massage
usually uses specific massage oils and essential oils with distinctive scents.

Traditional medicine and herbal formulations have been used by humans for
healing and treating various diseases and disorders. One of Indonesia's
cultural heritage is herbal medicine or commonly known as "Jamu"
(Indonesian Herbal Remedies). Jamu is the Indonesian traditional herbal
medicine that has been practiced for centuries in Indonesian community to
maintain good health and to treat diseases. Various formulations are made
based on a reference to the recipe of an ancient royalty cultural heritage, or
as a household remedy, or even known as a "folklore medicine" which were
commonly used.

Day 7 Empowering Community for Environment


Sustainability

People living in developing countries have different characteristics


compared to developed countries, such as in Indonesia. There is a high
sense of togetherness and kinship so that almost every region in
Indonesia has a local community such as RT (neighborhood-like) and RW
(community-like). With governance down to the community level,
Indonesia has an advantage in implementing environmentally conscious
programs for the community. Many environmental programs are
implemented using the bottom up method, one of which is community
empowerment. Some of these environmental programs include free
open defecation, waste management, and environmental health
programs.

To trigger the community to protect the environment, the City of


Surabaya held several environmental competitions such as "Merdeka
dari Sampah" and "Surabaya Smart City". This competition started in
2007 and is still ongoing until now. The Jambangan Village is one of the
winning villages in the competition, so that building environmental
awareness based on community empowerment is considered successful
in the City of Surabaya.

The existence of this environmental competition cannot be separated


from the role of the mass media. Several mass media in East Java
Province are located in Surabaya, one of which is Jawa Pos.This proves
that the mass media also plays a role in raising public awareness to
participate in managing the environment and to realize environmental
health.

Day 8 Observing Nursing Education Practice

CAM (Complementary and Alternative Medicine) or also called CT


(Complementary Therapy) is a useful practice to improve patient
wellbeing. Nurse could facilitate the delivery of such treatment since the
complementary medicine act as holistic care to maintain the high quality
of life for patient (Hori et al., 2008). Nurse act as the agent to improve
the care holistically. Patient in the hospital receive drug therapy or even
surgery type of treatment by the doctors, additional treatment which
proven beneficial should proposed to the health care team for the good
of the patient. The long-evidenced care that potentially benefit to the
patient is the CAM (Garland et al., 2013). The truth behind CAM, is that
patient using it before even get sick. Time and time again, research finds
that the majority of the population is using integrative health products,
and this rate is much higher for patients with higher socioeconomic
status and/or patients who suffer from chronic health conditions. The
role of nurse is to guide the use of CAM and even better, integrate the
use of CAM to improve the health outcome (Wojciechowski, 2020).

The theory of integrating the CAM is proposed by Dossey (2008) in his


theory of Integral and Holistic Nursing, she stated that the care for
patient is possible to be integrated with CT. The theory suggests that
there are four meta paradigm of nursing theory, which are person,
health, environment, and nurse (Admi, Eilon-Moshe and Ben-Arye,
2017). She stressed that nurse need to shifts the consciousness on
deliver care by incorporating holistic understanding and deepening the
level of meaning and understanding about knowing, doing, and being.
She proposed the idea of integral health and integral nurse, which is
curing patient is necessary to know the being. Nurse need to understand
what is the patient habit to achieve health (Dossey, 2020).

Complementary therapies are used to supplement conventional health


care practices. They include a wide range of treatment methods, such as
herbal therapies and manual healing, including reflexology and
acupuncture (College of Nurses of Ontario, 2020).

Work performed as part of a health service may use forms of


complementary and alternative medicine as one of the work-related
activities. Nursing Act 2014 No. 38 states specifically in subsection
Article 30(2) that "Nurses have the right to store work management
includes complementary and alternative while fulfilling public health
duties. Community Nurses must invite this matter in practice to use
forms of complementary and alternative medicine in providing
community services, both in home visits and in partnership work in
special groups and community work engagement (Susanto, 2019).

The use of CAM and its products, which are not currently considered
part of traditional medicine, has increased worldwide in recent years.
CAM has been used by diverse populations in the United States (53–
62%), European countries (20–50%), and Australia (52%) (TAKATA et al.,
2013). In the United States, the use of CAM methods such as
acupuncture, massage, healing, and yoga increased between 2002 and
2007(Yamashita, Tsukayama and Sugishita, 2002). Interest in and
demand for CAM is also growing rapidly in Japan. In the general
population, CAM use is reported to be up to 76%(Yamashita, Tsukayama
and Sugishita, 2002). The results of a study conducted in Japan showed
that 50% of patients took or used at least one CAM treatment in the last
12 months (Hori et al., 2008). The main reason for the increased use of
CAM is related to changes in the proportion of diseases such as B.
increase in chronic diseases, psychiatric diseases, malignancies and
unexplained diseases (TAKATA et al., 2013).

Integrative traditional health services are health services that combine


traditional health services with complementary traditional health
services. Traditional health services are carried out by health workers
and traditional health workers work together to care for
patients/clients. Integration Traditional health services must be
organized into health services. The Minister determines the types of
complementary traditional health services after receiving group
recommendations. The team consists of experts from the Ministry of
Health, Professional Associations, Doctors and Traditional Medicine
(Susanto, 2019). The Universitas Airlangga Education Hospital has
opened the complementary clinic which also the first hospital in
Indonesia who open this clinic. The hospital is gradually integrating the
service in the nursing ward. The nurses trained about the care
integration. The service in this clinic is acupuncture, acupressure, herbal
drink, massage, and many more (Ginanjar, 2022).

Day 9 Community Approach for Mental Health Issues

Lamongan is one of the regencies in East Java which has a fairly high case
of mental disorders. Based on RISKESDAS results in 2018, cases of
schizophrenia/psychosis were 7.31%, cases of depression were 3.24%
and cases of mental emotional disorders were 7.02%. The prevalence of
schizophrenia and emotional mentality in Lamongan district is higher
than the average prevalence in East Java Province, namely the average
prevalence of schizophrenia in East Java is 6.40% and mental emotional
disorders is 6.82%.

The prevalence and severity of mental health problems have increased


in the last decade. Globally, around 300 million people are affected by
depression, and 264 million are affected by anxiety. People affected by
mental health problems experience higher rates of disability and death.
In addition, mental illness causes lost productivity, increases the risk of
illness, and increases the risk of suicide. Globally, nearly 800,000 people
die by suicide each year. As we know, there are effective treatments for
depressive disorders, but 76-85% of people with depressive disorders do
not receive effective treatment, especially in lower-middle-income
countries. In Indonesia, only 9% of people with depression receive
medical treatment, while around 3.7% of the population has depressive
disorders. Depressive disorder is one of the five main causes of years
living with disability (YLD) in Indonesia with a percentage of 6.6%.

Integrated Service Post (Posyandu) is usually intended to support the


development of children. However, it is different from the Posyandu in
Lamongan, which was specifically established to assist people with
mental disorders (in bahasa Orang Dengan Gangguan Jiwa/ODGJ). In the
East Java region itself, especially in Lamongan Regency, with the current
pandemic there has been an increase of 31.1% new cases, and if these
mental disorders cases are not treated immediately. It will have an
impact on the level of difficulty recovering for mental patients.

The Posyandu Jiwa program, which is implemented by Lamongan


Regency as a first step in reducing existing mental disorders cases. The
purpose of program is to reduce the death rate for mental disorders due
to suicide. Mekar Sari Posyandu in Laren District, for example, has so far
managed to cure 114 ODGJ patients, called as Posyandu Jiwa
(Intergrated servive post for people with mental disorders). Even some
of them are now the backbone of the family. The most important
treatment and assistance for ODGJ is family support. In addition, routine
medication administration and communication with the social
community is very important. In the past, many people thought that
ODGJ was a disgrace to the family. But slowly we changed and believed
in this activity in Laren Health Center. Because there is Posyandu Jiwa in
health center in Lamongan, particularly in Laren Health Center, now
people who suffer mental disorders now is getting better and many are
recovering.

PURPOSES

health promotion in Lamongan, East Java

Day 10 Collaborating Health Education in Asia Pacific

Collaborative education through new approaches that enrich our health


programs and extend into practice. Focus areas not only about health
education, practitioner and system development. New Colombo Plan
has accomodated meeting between academics and external partner.
Through the Collaborative Health Education we hold guest lecture, we
aim to foster a culture of purposeful collaborations to advance our
strategic objectives of education innovation, expanded placements, and
an education ecosystem of team-based care.

Recognizing that collaborative health education can only be progressed


through a combination of internal facilitation and external
mobilization—with faculty members and academic units playing a key
role . By bringing together interested partners around educational issues
of common concern, this event will facilitate knowledge exchange and
synergies , mobilize and coordinate partners , create an impetus for
internal collaboration

Each collaborative health education will drive continued


transformations in teaching and learning across health and health-
related programs, in both classroom and clinical settings, to help foster
