Task Objective
In this activity, you should be able to use word processors to produce a questionnaire for
the students in your school and to summarize the results.

2. The group will be given time to prepare the questions that will be distributed to the
student body for the survey.
3. The group will compile all the answers and prepare a summary of the survey using a
word processor.
4. The group will print the output and submit it to their teacher.

Activity Output
You are a group of students who would like to know the opinions of your fellow
schoolmates on a specific topic. You have decided to create a survey that will be distributed
to different grade levels in your school. Your target is to get the feedback of at least 25
students. You should create at least 10 questions for your survey.

Use the table below for your planning and idea brainstorming. Below are the topics that
you can choose from in creating your survey. You can think of your own topic and ask
permission from your teacher.
