    LETTER OF EXPLAINATION

    Good Day Sir!

    I am writing this letter to request for another copy of form 137, CAV form 4,
    CAV form 5, and CAV form 17. The reason why I have to request for these
    documents again, is because the Regional Office X want it to be computerized and
    they have already expired.

    The Regional Office X also want my daughter Jacqueline Janolino to get a


    Special Power of Attorney (SPA) from where she is living abroad. For this reason,
    the envelope containing the said documents was stranded in our home. It was placed
    near our window and it was unfortunately drenched by the strong rain. For the
    intention of saving it, I opened the envelope to dry the documents.
