Madam:

Joyful greetings!

I, LALAINE D. CABALTEA, Teacher II and a Kindergarten teacher of Antipolo Heights

Integrated School, Pasacao District and have been teaching for four (4) years and nine (9)
months.

This is to express my intent to seek for transfer from the present school, Antipolo Heights
Integrated School to Mandacanan Elementary School or any nearby schools in Libmanan North
District with the following reasons in hand,

1) I am residing at Zone 5, Apad, Potot, Libmanan, Camarines Sur which is 7-10 minutes
away from Mandacanan Elementary School.

2) The delivery of my son was through cesarean section (CS) on November 21, 2021 that is
why my parents and my husband are anxious and worried for my safety especially during
rainy season.

3) I am a nursing mother of my son, 1 year and 5 months old.

4) The distance of our place to my station hinders my commitment to serve. And I already
suffered illnesses due to almost 2 hours travel going back and forth to school riding with
jeepney and “habal-habal”.

5) I experienced putting my life in risk. Antipolo Heights Integrated School is in a far-flung

area, where the means of transportation is by a motorized vehicle that during rainy season
our way home is risky due to slippery road and flood.

6) To economized my weekly expenses for my fare in commuting from my residence to my

station and other expenses.
7) I am 12 weeks pregnant and I am worry for my safety and the baby in my womb.

In view of the above mentioned reasons, and in consideration of my right and privileges, please
understand my situation and I am humbly appealing to your office to consider my intention to
transfer. Thank you very much and God bless.
