AFFIDAVIT OF EXPLANATION

I, TREXTER PINEDA OSORIO, of legal age, single, Filipino, and presently

residing at Block 12 L12 Quirino St., Ph11-A, Bahayang Pag-Asa Subd., Bgy. Pasong
Buaya 2, Imus City, Cavite, Philippines, after having been duly sworn in
accordance with law, do hereby depose and say;

That I was the holder of Philippine Passport No. EB77616271 issued by the
Department of Foreign Affairs, NCR West on 01 April 2013 which shall
expired on 31 March 2018;

That the said passport was used for work abroad in Riyadh Saudi Arabia as
Coordinator;

That the said document was burned when a fire transpired in the above-
mentioned place on May 31, 2021;

That I am executing this affidavit in order to attest to the truth of the

foregoing facts and for all legal intents and purposes this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 29th July 2021, in
the City of Las Piñas, Philippines.
