having been duly sworn to in accordance with law, depose and state:

1. That I am the actual occupant, possessor of a parcel of land identified as Lot No.
___, Cad _______, with an area of TWENTY SEVEN (27) SQUARE METERS,
situated at Purok 5, Jalajala, Rizal;

2. That I have been in actual open, adverse and continuous and possession of the
above-mentioned parcel of land up to the present;

3. That I have introduced considerable improvements thereon including house made

of light materials where my family actually used;

4. That I am executing this Affidavit to attest to the truth of the foregoing

statements and for whatever legal purposes this may serve.
