PROPOSED RESOLUTION NO. 1

SERIES OF 2024

A RESOLUTION REQUESTING FINANCIAL ASSISTANCE TO HON. JUAN R. CAPUCHINO, MUNICIPAL

MAYOR OF NAGUILIAN, ISABELA FOR THE RENOVATION OF BARANGAY HEALTH CENTER OF
BARANGAY CABARUAN, NAGUILIAN, ISABELA

“WHEREAS, it has been agreed by the Sanguniang Barangay Members that the Barangay Health Center
was damaged and in need of total renovation.”

“WHEREAS, the Barangay Hall was already damaged due to the disasters that have passed and the
period of time it has been built.”

“WHEREAS, improving the Barangay Health Center is needed to speed up and improve the service for
the constituents of the Barangay.”

“NOW THEREFORE, on motion of Hon. Primitivo D. Dacanay duly seconded by Hon. Jornald D. Paguyo
and the rest of the body, be it.”

“RESOLVED, it is hereby resolved to make a humble request to allot funds needed for this project.”

“RESOLVED FURTHER, the copies of this resolution shall furnish to the other concerned for their
information appropriate action.”
