CAT-Z LOTO SPECIFIC PROCEDURE, RISK ASSESSMENT & CONTROL MEASURES - PRCS RECLASSIFICATION - STEAM

TUNNEL
Equipment/Type/Name: Steam Tunnel Manufacturer: Lenoard Vision
Model/Description/Number: V35GH Serial Number: V10812GC
Only AUTHORIZED partners are allowed to perform LOTO.
All OTHER/AFFECTED partners must not attempt to start, energize, or use this machine while it is locked out.
Partners who do not comply with this procedure will be subject to disciplinary actions and/or termination.
Select All Hazardous Energy Sources for Equipment Select All Potential Stored Energy for Equipment
Electric 460/480V x Steam Air pressure
Electric 208/230/240V x Gas x Steam Pressure
Electric 120V x Stored Electricity
x Other Electric x Thermal Heat
Compressed Air Chain or Belt Tension
Special Instructions Fully understand the scope of work to be performed before starting.

All energy sources selected above shall be isolated, tried and locked prior to performing
work/maintenance on this equipment/task. If work is performed while either the equipment is elevated or
a piece of the equipment is positioned such that it presents potential stored energy, all potentially
hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered
1) Isolation
safe. All hazards exposed by equipment/components/doors in a raised state, items with elevated heat,
capacitors, drive mechanisms, tension to belts/chains/springs and any other potentially hazardous stored
energy must be rendered safe before performing work.
