Background of the Activity:

The Cluster XI Secondary Schools will conduct its SSLG Leadership Training on January
19-20, 2024, at Marilog National High School. This is a two-day and one-night activity that aims
to capacitate the 2023-2024 SSLG officers' leadership skills.

By signing this parental consent, we hereby willingly and voluntarily give consent to the participation
of our son/daughter _________________________________________ in the Cluster X1 SSLG
Leadership Training as SSLG Officer to be held at MARILOG NATIONAL HIGH SCHOOL on
January 19-20, 2023 (OVERNIGHT).
I have considered the benefits that my/our son will derive from his participation in this activity
provided that due care and precaution will be observed to ensure the comfort and safety of my/our son
and that DepEd employees and personnel may not be held responsible for any untoward incident that
may happen beyond their control.
