AFFIDAVIT OF UNDERTAKING & WAIVER OF LIABILITY

I, RODOLFO S. MADRELINO JR. of legal age, Filipino with residence and postal address at Zone 5 San Jose, San
Miguel, Iloilo. After having been duly sworn to in accordance with the law, hereby depose to say.

1. That, I am the authorized representative of Bricolage Philippines Inc. (MR.DIY) located at Yati, Liloan Cebu,
Philippines.

2. That I/the company have an application for Building Permit, Fire Safety Inspection Permit and Certificate of
Occupancy.

3. That the said application is currently on process before the Department of the Building Official and Bureau
of Fire Protection.

4. That I/the company undertake to secure the necessary pending permits within Ninety (90) days from the
time of this undertaking was sign.

5. That I/the company hereby waive the Liloan, Cebu LGU, or any its officers, directors, employees, and
representatives from any and all claims, liabilities, demands, actions, causes of actions, cost and expenses,
whether at law or equity, arising out of or in connection with this the opening of our store.

6. That this Affidavit is executed in order to attest to the truth of the above-mentioned facts and for whatever
legal purposes it may serve.
