Safety Risk Management

A hazard is an event, condition, or circumstance


that can lead to a loss
Hazard Identification
• Hazard identification is vital for safety

• Hazards pose risks, and if you do not know what the risks are, you
cannot do anything about them

• Some hazards can be identified from occurrence or incident


reports, some may be reported by individuals, and others may be
the result of proactive brainstorming sessions

• Encourage everyone to report any hazards they come across (as


small as they might seem)
Hazard Identification
• Use a Hazard Log to record the hazards people find or think of-
• It can be very simple
• It might be a notebook in the crew room or a sheet on the notice board
• It should be easily accessible and visible; it is for everyone
• Encourage people to comment on the safety issues others have logged

Hazard Identification

• It’s important to understand that-


• A hazard is not the consequence of a risk,
rather;
• Risk is the consequence of a hazard

• Birds near the airport are a hazard

• FOD damage is the risk


(consequence) of the hazard
Risk Assessment
• You will need a process to identify
what could happen as a result of
each hazard (the consequence)
and assess how bad the outcome
will be and the likelihood of it
happening (the risk)
• Can you accept the risk? If not,
you must do something to reduce
it (control or mitigation)-
• A risk matrix may be useful, but in a
Risk Matrix
Very Small organization, it may not
be necessary
Risk Assessment
• The summary of the risk assessment should be documented in
the Hazard Log. This should include the risk assessment for each
hazard and any actions required to control the risk

• Validation, monitoring, and feedback-


• Monitoring provides a method to know if the safety risk system is working

• Understand that new hazards/risks may arise in the future and


may have to be analyzed and added to your list
