Blessed day!
I, EVEYOLLY R. FETALCO, a resident of Blk 11 B Lot 12, Casa de
Monteverde, Brgy F. De Castro, GMA, Cavite, would like to request from your
good office the issuance of Manuel Tubaying’s title. That was scheduled
supposedly last July 10,2023. Unfortunately, I was not able to go there personally
since i was confined in the hospital that day of my appointment with you.. And i
learned Miss Jackie who attended on us during the transaction period was also on
leave . However, I requested my husband to do it in my behalf as/ complete
document with him but your office refused to entrust him that said title.
In this connection, I am requesting for another schedule in releasing this
title and if possible it would be on the 23rd of august 2023 .I am requesting also to
waive the penalty
