CONTRACT OF SERVICE
KNOW TO ALL MEN BY THESE PRESENTS:

This contract is made entered into by and between Department of Education, SAN AGUSTIN
ELEMENTARY SCHOOL, with office address at San Agustin, Carmen Agusan del Norte,
represented by BERNIE R. PAMPLONA, School Head , hereinafter referred to as the “First
Party”.

And

____________________ of legal age, Filipino, resident of ___________________________,


Agusan del Norte hereinafter referred to as “Second Party”.

WITNESSETH

1. That the First Party needs the service of the Second Party who shall perform the
services in decorating and materials in the School Stage during the Year-End
Graduation and Moving Up Ceremony on July 10&11, 2023;
2. That the Second Party has signified his/her genuine intention, by providing the
services and materials needed by the First Party;
3. That the Second Party possess the minimum requirements in terms of experience,
education and skills as an essential requirement for the performance of his job;
4. That the First Party offers to the Second Party the sum of money in the amount of
Five Thousand Eight Hundred Ninety Pesos Only (5,890.00) as payment for the
materials and services rendered;
5. That the First Party will only be paid for the services and materials after the conduct
of the Activity.
6. That the Second Party shall perform his/her assigned work based on the agreed time
and scheduled by both parties.
