Activity: Grade 7 – Alguno 1st Homeroom PTA Meeting

Date & time: June 21, 2019, 08:00 am
Venue: Vinapor NHS

I. CALL TO ORDER
The meeting was called to order at 09:00 am by Ms. Grade 7 – Alguno adviser, Ms. Kristel C.
Saluta. Mrs. Normanita Cagampang led the prayer.

II. AGENDA OF THE MEETING:

1. Election of Homeroom PTA Officers
2. School’s Rules & Regulations
3. Form137
4. NSO Birth Certificate
5. K to 12 Grading System
6. PTA Project
7. Other Matters

III. Meeting Proper

1. The following is the agreed set of Homeroom PTA Officers for school year 2019-2020:
President: Normanita L. Cagampang
Vice President: Jens Marjorie Uriarte
Secretary: Gina B. Adellon
Treasurer: Modesto C. Montecillo
Auditor: Merna Ortiz
P.I.O: Jayson V. Senamin and Apolinario A. Mapilit
Muse: Janet P. Pasaylo
Prince Charming: Reynante P. Diamada

2. Ms. Saluta reiterated the following school’s rules and regulations:

 The school never allows boys to have long hair, in short, they should always observe proper
haircut.
 Students should wear their uniforms every day except for Wednesday.
 On Wednesday, students will wear jeans and plain white t-shirt or they can use their WATCH
t-shirts if they have it.
 Student’s tardiness and absences will be recorded by the class monitor so that in case the
parents would want to know the status of their children, there is a record that serve as basis.
3. Ms. Saluta reminded the parents to follow up their form 137 or sf 10 from their previous school.
4. Ms. Saluta also reminded to submit the student’s NSO Birth Certificate.
5. Ms. Saluta informed the parents about the grading system. For her subject Mathematics, below is
the breakdown:
Written works: 40%
Performance: 40%
Quarterly Assessment: 20%
Total: 100%
6. The body agreed that the PTA Projects for school year 2019-2020 is wall fan. In order to realize
the project, the body agreed that each student shall contribute 70.00 and the deadline will be on July
19, 2019.

The meeting was called to order at 02:00 am by Ms. Grade 7 – Alguno adviser, Ms. Kristel C.
Saluta. Mrs. Uriarte led the prayer.

II. AGENDA OF THE MEETING:

1. Students Status in First Grading Period
2. Form137
3. NSO Birth Certificate
4. Giving of Certificates for the Outstanding Students
5. Giving of Card to the parents
6. Other Matters

III. Meeting Proper

1. Ms. Saluta inform those parents who have problematic students. Ms. Saluta mentioned the
students who are always tardy and lot of absenteeism. Ms. Saluta warned those parents whom their
students have more than 5 consecutive days without notice of absent are prone to dropped out.
2. Ms. Saluta reminded the parents to follow up their form 137 or sf 10 from their previous school.
3. Ms. Saluta also reminded to those who were not yet submitted the student’s NSO Birth Certificate.
5. Giving of certificates for those deserving students of Grade 7 – Alguno.
6. For other matter, Ms. Saluta give thanks to all parents who supported the classroom project for
School Year 2019-2020. The classroom project is finally realized with the help of the parents.
