4. Mrs. Tejero reiterated the following school and classroom’s policies:

 Pupils should wear their school uniform from Monday to Thursday. On Friday it’s their wash day.They
can wear formal dress or white t-shirt. On Wednesday, they can wear their P.E. uniform.
 Pupils should wear their school I.D. in coming to school.
 Pupils’ tardiness and absences will be strictly recorded for these will be the basis on giving award for
perfect attendance per quarter.
 Pupils’ should be in school before 7:15 in the morning and to attend the flag raising ceremony. Time of
departure in the morning is 11:00.In the afternoon session, classes will start at 1:00 and will end at 3:40.
 Pupils who have “sundo” should stay in the designated area where the parent agreed to fetch them or
they can stay in the classroom.
 Buying outside the school premises is strictly prohibited. They can bring their baon or to buy in the
school canteen.
 Gate pass will be given to the pupils if they will go out from the school during class hours.
5. Mrs. Tejero reminded the parents of the transferees to get the form 137 of their children from their previous
school using the Request Form which will be given after the meeting and those parents whose children have
not submitted their NSO Birth Certificate to provide the teacher a copy.
6. K to 12 Grading System was presented and explained to the parents:

7. Other Matters:
a. The teacher required every pupils to have notebook for every subject labelled with the subject and their
name.They must also bring their complete Art envelope and a Portfolio Envelope.
b. The parents agreed to contribute P50 for the drinking/mineral water
c. One week after the rating period, the adviser will call again a meeting to report to the parents the
progress of the pupils and this will be also the signing of the report card and test results.
d. For the maintenance of the ground assignment, since grade II pupils could not do it religiously every
morning, the parents agree that they will hire somebody to clean the ground assignment and they will
pay P1000 for her every month. So each parent will contribute P322 for the whole year as salary of the
cleaners
 e. Wellness program is given during flag ceremony
f. Reading program for those slow readers will be given during remedial instructuion
g. Books to be distributed to the pupils but the teacher asked favour from the parents to have these repair
and covered.
h. MTAP Review for the contestants will be done after class hour.
i. Have the pupils bring extra tshirt and towel in coming to school.
j. Cellphone and toys are not allowed to bring to school by the pupils.
k. Teacher reminded pupils to always remind their children to behave always in the school.

Proceedings:

1. The meeting was called to order at 2:00 PM by the adviser, Mrs. Carlota A. Tejero.
2. Report card was distributed to the parents for signature.
3. Pupils assessment results were also given to the parents for signature and to see the performance of their
child for the grading period.
4. Mrs. Tejero reiterated again the effect of missing the class for even one day to the performance of their
child.
She again explained the performance task which has a high percent contribution to the grades of the pupils.
5. The teacher announced the schedule of the 1 st grading awarding ceremony.
6. The Teacher presented one of the problems of the classroom which was the damage wall behind which is
destroyed by the termites. She asked donation from the parents for the repair. The parents agreed to give
donation for the repair.
7. For other matters, she reminded the parents for the PTA Fee of P100 and School Paper of P60 per parent.
She also reminded the parents about the collection for the payment of the cleaners.
8. The meeting ended at 3:00 PM.

Date: November 17, 2018

Proceedings:

1. The meeting was called to order at 3:00 PM by the adviser, Mrs. Carlota A. Tejero.
2. Report card was distributed to the parents for signature.
3. Pupils assessment results were also given to the parents for signature and to see the performance of
their child for the grading period.
4. Mrs. Tejero explained that some pupils got lower grade in other subjects because they lack focus.
They were inattentive which resulted to getting low score in the weekly test and summative test.
5. For other matters, the teacher reminded the parents about the food safety of their children. Instead
of giving money they can have them bring baon from home like rice and viand and simply water will
do.
6. She also reported the incident in which 5 pupisl went outside the school during recess time to buy
something from the CO STAR without asking permission from the teacher.
7. The meeting ended at 4:00 PM.

Date : February 2, 2019

Proceedings:

1. Third grading awarding ceremony was held in the morning at 8:00 AM in the Function Hall.
2. The meeting was called to order by the adviser, Mrs. Tejero at 3:00 PM.
3. She distributed to the parent the report card and learners’ assessment results for signature.
4. She explained that there were honor pupils from the 2 nd grading period who were not included in the 3 rd
grading awarding because they did not get the cut off final grade of 90%. She encouraged parents to follow
up their pupils at home especially in their homeworks and activities.
5. For other matters, she again reminded other parents for the cleaner’s payment. She also mentioned about
the scarcity of water supply which affected the use of the toilet. She informed the parents that she got
amount from mineral water collection to buy water for the toilet use.
6. The meeting ended at 4:00 PM.

Proceedings:

1. The meeting was called to order at 3:00 PM. by the adviser, Mrs. Carlota A. Tejero.
2. Report cards and learners’ test results were given to the parents for signature.
3. The teacher presented on the board the list of pupils who have honors for deliberation. Basis for the ranking
is purely academic. Extra-curriculars were not included although giving grades to the contestants were
considered in the computation. Four pupils got with high honors and 9 got with honors. MTAP contestants
will also be given a certificate of appreciation.
4. Recognition day will be on April 5 at 8:00 am.
5. Finances for the recognition day were presented during PTA meeting in the morning of this day with the
parents of honor pupils. The facilitator of the meeting, Mrs. Tejero,reiterated that no available funds for the
Recogniton day activities. Parents were asked for donation.
6. For Grade II parents, they were able to give P3,300.00 donation.
7. The meeting ended at 4:00PM.

Proceedings:

1. The meeting was called to order at 2:15 PM. by the adviser, Mrs. Carlota A. Tejero.
2. New class schedule was presented to the parents. Start of flag ceremony is at 7:05, classes start at 7:25
and ends at 11:25 and in the afternoon classes starts at 1:00 and ends at 2:30 to 3:00.
3. Recess time will be at 9:35. The teacher suggested that pupils shall bring rice and viand for baon.
4. The teacher suggested that during the weekly test pupils shall answer on the test paper but she said that
it would be just ok if the parents shall pay for the Xerox copy of the test papers. The parents agreed to
the suggestion of the teacher.
5. Wins program was discussed by the teacher and each pupil shall bring toothbrush with toothpaste, hand
towel and soap. Deworming will be done on the following week and pupils should bring baon in the
morning.
6. The teacher said that there were some pupils who are slow readers and therefore, parents shall have
follow up at home. The teacher also shall conduct remedial reading at 2:30 to 3:00 PM.
7. Other matters:
 Pupils shall bring their own balunan for water refiller.
 Pupils are requested to bring chalk for their individual activity using slate board.
 Parents were asked if they receive the text messages of the teacher.
