AFFIDAVIT–CUM–DECLARATION

We, 1) MR. CAJETAN PHILLIP DCOSTA – Aged about 61 Years, (Aadhar


Card No. 388123478857, PAN No. AHUPD7305C) 2) MR. ALLAN CAJETAN
DCOSTA – Aged about 28 Years, (Aadhar Card No. 379161919350, PAN
No.BAYPD384H), both residing at H/5, Flat No.302, Geetanjali CHSL., Ambadi Road,
Dewan Sons, Nr. Municipal Swimming Pool, Vasai (West), Palghar - 401202, adult, Indian
Inhabitants of Mumbai, having address as stated above do hereby jointly and severally state
and declare on solemn affirmation as under: -

We, state and declare that the subject residential tenement viz. Room No. B-609,
admeasuring 300 Sq. Ft. in German Darshan CHSL., located on CTS No. 367, at Tardeo
Road, Tulshiwadi, Mumbai - 400034, is allotted to Mr. FRANCIS RODRIGUES, vide
allotment letter bearing No. 1417/2019 dated 25.07.19, issued by Mumbai Building Repair
And Reconstruction Board, and that he has lawfully procured the said residential premises
being its bonafide owner, having Electric Consumer Account No. 748-210-085*0.
We, state that the said Mr. FRANCIS RODRIGUES, is related to Deponent No.1
as his FATHER-IN-LAW and that he is related to Deponent No.2 as his GRANDFATHER.
And that the said Mr. FRANCIS RODRIGUES, being fully seized and possessed of the
said premises, is entitled to the said residential premise without any hindrance and or
objection from anybody.

We the Deponent No.1 & 2 state that being legal heirs to MR. FRANCIS
RODRIGUES; we are aware that he intends to transfer sell and convey rights, title and
interest in residential tenement viz. Room No. B-609, admeasuring 300 Sq. Ft., in German
Darshan C.H.S.L., unto and in favour of MR. HEMANT KHANDUBHAI KASLA,
perpetually. And that on such subsequent transfer/conveyance of rights, title and interest in
residential tenement viz. Room No. B-609, the said residential premises
shall stand vested in the name of MR. HEMANT KHANDUBHAI KASLA, in the
records of concerned, Society, Mumbai Building Repair And Reconstruction Board or any other
Govt. or Semi Government authorities and that we shall not raise any objection for transfer of
the aforesaid residential premises and its 100% share in the name of MR. HEMANT
KHANDUBHAI KASLA.

We vide present instrument hereby undertake to sign and execute all required documents
for transfer of the aforesaid residential premises in the name of MR. HEMANT
KHANDUBHAI KASLA in the records of concerned society, Mumbai Building Repair and
Reconstruction Board and/or concerned Authorities.

We hereby undertake to indemnify and keep indemnified the said society, Mumbai
Building Repair and Reconstruction Board, concerned authorities against any adverse claim by us
and/or anyone claiming for and on our behalf to such transfer of the said residential premises in
the name of MR. HEMANT KHANDUBHAI KASLA, from the name of MR. FRANCIS
RODRIGUES.

We are making this Affidavit to show the abovesaid facts and in order to produce it before
the concerned Authorities for transfer of the aforesaid residential premises in the name of MR.
HEMANT KHANDUBHAI KASLA, from the name of MR. FRANCIS RODRIGUES.

Whatever stated hereinabove is true and correct to the best of our knowledge and belief
and if the contents of this Affidavit are found false in future then we shall be prosecuted for the
same.
