Dear Elvis,

RE - OFFER LETTER OF CONTRACT

Following your recent successful interview at Kaiser Global Health Limited, we are pleased to
inform you of our decision to appoint you to the position of Sales Representative (Kumasi Office)
within Sales and Marketing Department. The offer of contract is for a period of three (3) months
from August 13, 2019 (‘Effective Date’) and shall continue in pursuant to the terms and conditions
set forth in this offer letter and employment contract until the close of business November 13, 2019
(‘End Date”) with the possible option to extend solely based on performance and organizational fit.
This offer do not guarantee full time employment with KGHL .This offer is for your review and
acceptance.

Your monthly target or closed sales leads (subject to review quarterly and annually) are as follows;

 Ten (10) families signed up to KGHL family plans or

 Twenty (20) individuals signed up to KGHL Ayarisa plan.

With regards to your compensation, please note that Kaiser Global Health Limited will be offering
you the following:
a) Gross Salary: Your consolidated monthly allowance will be Ghana Cedis Five Hundred Only
(¢500.00) subject to statutory deductions and local tax laws (withholding tax of 7.5%) which shall
be payable monthly not later than the 29th day of each month. Breakdown as follows;
Allowance (Monthly)
Basic Salary – GHC 500.00

b) You shall be entitled to KGHL approved commission rate below on all new businesses that you
are able to bring or sign onto Kaiser Global Health Limited products namely Corporate, Ayarisa
and Family respectively and also on your renewals of these businesses only if monthly targets are
exceeded.

By accepting this offer, you agree that throughout your contract, you will observe all policies and
practices governing the conduct of our business and employees, including our policies prohibiting
discrimination and harassment and signing oath of confidentiality.

Please note that a full contract will be issued to you detailing the full terms of your employment
agreement with Kaiser Global Health Limited.
