We are pleased to offer you employment with Customer Satisfaction Corp., the Philippine
branch (CASA DE CUSINA) under the following terms and conditions:

(1) Your official designation in the Company will be Casa De Cusina – Line cooks - Coordinated
customer satisfaction and you will be expected to perform the job duties as described in the
attached Job Description.

This position is for full time employment. You shall work forty (40) hours a week or such hours as
determined by the Company based on a schedule assigned by your immediate manager. You must be
willing to work flexible hours and on short notice. The Company operates twenty-four (24) hours a
day, seven (7) days a week, and may require you to work on any given shift. Furthermore, the
Company may require you to work in a modified workweek to accommodate operations. You
shall be compensated for overtime, night differential, and holiday pay in accordance with the
Philippine Labor Code. You also agree that the Company has the right to change your duties which
shall include but not limited to: transfer or re-assignment to its related companies, different clients,
branched as it deems necessary and depending on your qualifications and customer’s demands.

Should you choose to accept this position, your start date will be on January 5 , 2023.

(2) For the services you render, you shall receive a base salary and benefits set forth as line cook.

(3) Your employment will be on probationary basis for a period of six (6) months or 180 calendar days
beginning Start Date and ending, July 2023 and will be subject to the Company’s standards for
regularization of employment as described in your Employment Contract.

(4) This Offer of Employment is contingent upon your completion of Pre-Employment Requirements
(including the positive results of the Pre-Employment Medical Examination Submission of the
above requirements entails the collection and processing of your personal information for
purposes of your employment. When you submit such requirements, please be guided by C3’s
Data Privacy Statement (Annex “D”).

(5) Upon signing and acceptance of this Offer, you are required to:

a. submit this signed Offer to Casa de Cusina Human Resources Department


b. provide a valid proof of identification and, if you are a foreign national to HR, a valid
authorization to stay in the Philippines;

c. submit and/or comply with Onboarding Requirements in Casa De Cusina

This employment offer will remain valid for 3 days from date of issue, and is subject to the
following conditions:
(a) fulfillment of the conditions mentioned in this offer; (b) your acceptance and
delivery of the Onboarding Requirements and other documents required by the
Company; (c) successful completion of all aspects of your application process,
which includes passing pre-employment medical examinations, drug testing and
reference checks; and (d) signing of the employment contract.

FOR FOREIGN NATIONALS ONLY

This Offer is also conditioned upon the issuance of the visa and work permits by the Bureau of
Immigration (BI) and Department of Labor and Employment (DOLE) and other relevant government
authorizations (collectively, “Work Authorizations”) which may be necessary for your lawful
employment with the Company in the Philippines. It shall be considered automatically withdrawn when
the Work Authorizations are not obtained from or released by the government authorities for
whatever reason.

If you are a foreign national, you further agree that your continued qualification for employment is
based on a favorable grant by the Philippine Government of your Work Authorizations. Should your
Work Authorizations be revoked, cancelled or changed at any given time, for any reason whatsoever,
this Agreement shall be deemed terminated in accordance with law.

COMPENSATION AND OTHER BENEFITS

(1)Your gross basic salary will be Php 21,000.00 per month (“Basic Pay”), which is payable on a semi-
monthly basis or every 15th and 30th of the month, which schedule may be amended in accordance with
Labor Laws. Your Basic Pay will be subject to the appropriate withholding tax, and SSS, PhilHealth, Pag-IBIG
Fund contributions, and other authorized deductions. It is guaranteed for 13 months in a full calendar year;
however, the 13th month Basic Pay will be prorated depending on your Start Date or termination date in the
relevant year.

(2)Your regular allowance is Php 1,250.00 per month, on a twelve (12)-month basis per calendar year,
payable semi-monthly. This allowance will be dependent on your attendance, and pro-rated deductions
will be made in cases of absences or leaves or for any days when you do not work or there is no work,
whether the absences, leaves or no-work days are paid or unpaid in accordance with the Company’s
Attendance Policy.

(3)Your total gross monthly compensation will be Php 22,250.00, which is the total of your gross monthly
basic salary and allowances, subject to the adjustments upon Account Certification (if applicable).

(4)You shall be entitled to the following additional benefits:

a.HMO benefit for you and, upon regularization, HMO coverage for 1 dependent.1
b.Bereavement Leave of 5 days per year which may be used within a reasonable time from the death of an
immediate family member in accordance with the Company’s Leave Policy (“Leave Policy”).2
c.Personal Leave of a total of 15 days per year, earned monthly on Start Date with eligibility of utilization
upon regularization in accordance with the Leave Policy.
d.Group Life Insurance
